2018-2019年医院中药饮片、中西药联用住院医嘱点评与分析

Comments and analysis of inpatient doctor's orders for Chinese herbal medicines and combination of traditional Chinese and Western medicine in hospital from 2018 to 2019

目的:点评2018-2019年某院中药饮片、中西药联用住院医嘱,分析不合理的原因。方法:选取2018年1月-2019年12月住院病例共720份,进行处方点评。结果:分析720份住院病例,发现存在问题病例156份(21.7%)。存在应用中药饮片无辨证证型、无中药脚注、超剂量使用毒性中药饮片、中药饮片生熟选用不合理、存在药物配伍相互作用、重复用药应用及中药未体现辨证施治等问题。结论:某院住院中药饮片使用存在问题偏多,通过处方点评可提升医院用药的有效性和安全性,保障患者用药安全。

Objective:To comment on the inpatient doctor's orders of Chinese herbal medicines and combined use of Chinese and Western medicine in a hospital from 2018 to 2019,and analyze the unreasonable reasons.Methods:From January 2018 to December 2019,a total of 720 hospitalized medical records were selected and evaluated.Results:720 hospitalized medical records were analyzed,and 156(21.7%)were found to have problems.There are some problems in the application of prepared pieces of traditional Chinese medicine,such as no syndrome differentiation,no footnotes of traditional Chinese medicine,over-dose use of toxic prepared pieces of traditional Chinese medicine,unreasonable selection of raw and mature prepared pieces of traditional Chinese medicine,interaction of drug compatibility,repeated application of drugs and non-dialectical treatment of traditional Chinese medicine.Conclusion:There are many problems in the use of prepared pieces of traditional Chinese medicine in a hospital.Prescription comments are used to improve the effectiveness and safety of drug use in the hospital to ensure the safety of patients.
