医院-社区联动干预对慢性伤口患者延续性护理的研究

Study on continuous nursing care of patients with chronic wounds by hospital-community linkage intervention

目的:探讨医院-社区联动干预在慢性伤口患者延续性护理方面的应用价值。方法:2018年2月-2019年2月收治慢性伤口(以压力性损伤为主)患者100例,采取医院-社区联动干预的延续性护理模式,评价护理效果及社区护理人员培训效果。结果:采取延续护理后,较护理前创面面积及疼痛评分均显著下降,差异有统计学意义(P<0.05);患者护理服务满意率达100.00%;社区护理人员培训后,较培训前创面健康教育知识考核、护理技能考核及慢性伤口专业知识评分均明显提高,差异有统计学意义(P<0.05)。结论:医院-社区联动干预对慢性伤口患者开展延续性护理,有助于为社区培养慢性伤口专科护理人员,提高社区对慢性伤口的处理能力,更好地服务社区患者。

Objective:To explore the application value of hospital-community linkage intervention in the continuous care of patients with chronic wounds.Methods:From February 2018 to February 2019,100 patients with chronic wounds(mainly stress injury)were selected,the patients received the continuous nursing mode of hospital-community interaction intervention,we evaluated the nursing effect and the training effect of community nursing staff.Results:After continuing nursing,compared with before nursing,the wound area and pain score were significantly decreased,the difference was statistically significant(P<0.05).After training of community nursing staff,compared with before training,wound health education knowledge assessment,nursing skills assessment and chronic wound professional knowledge scores were significantly improved,the difference was statistically significant(P<0.05).Conclusion:Using hospital-community linkage intervention to carry out continuous nursing care for patients with chronic wounds can help to cultivate specialized nursing staff for chronic wounds in the community,improve the ability of community to deal with chronic wounds,and better serve community patients.
