摘要
目的探讨涎腺肌上皮瘤的病理特征与免疫表型的关系。方法回顾性分析34例涎腺肌上皮瘤的临床病理特征,采用免疫组化EnVision两步法检测肌上皮细胞标志物CK(AE1/AE3)、Calponin、S-100、SMA、SOX-10、vimentin的表达情况。结果34例涎腺肌上皮瘤中,上皮样型11例(11/34,32.3%),梭形细胞型13例(13/34,38.2%),浆细胞样型7例(7/34,20.6%),透明细胞型3例(3/34,8.8%);上皮样型中上皮性标志物CK(AE1/AE3)和肌源性标志物SMA和Calponin表达最强,梭形细胞型和浆细胞样型对肌源性标志物表达强于上皮性标志物,透明细胞型对上皮性标志物和肌源性标志物表达最弱。SOX-10在34例涎腺肌上皮瘤中均呈弥漫阳性且表达强度在各种肌上皮细胞间无显著差异。结论涎腺肌上皮瘤的肌上皮形态与免疫表型具有相关性,SOX-10作为一种肌上皮细胞标志物具有较高的敏感性。
Purpose To investigate the pathological features and immunohistochemical patterns of salivary gland myoepithelioma.Methods A retrospective analysis was carried out in 34 cases of salivary gland myoepithelioma using myoepithelial markers including CK(AE1/AE3),calponin,S-100,SMA,SOX-10 and vimentin.Results 34 cases of salivary gland myoepithelioma were further classified into epithelioid type(11/34,32.3%),spindle type(13/34,38.2%),plasmacytoid type(7/34,20.6%)and clear cell type(3/34,8.8%).CK(AE1/AE3),SMA and calponin showed high expression level in epithelioid type,compared with the expression of epithelial markers,spindle type and plasmacytoid type were detected with stronger expression of SMA and calponin.Meanwhile,clear cell type showed relatively lower expression of CK(AE1/AE3),SMA and calponin.Remarkably,SOX-10 was well detected in all 34 cases of myoepithelioma while the expression was similarly strong in all myoepithelial type.Conclusion There is a correlation between the morphology of myoepithelioma of salivary gland and immunohistochemical expression,and our current data suggests that SOX-10 is applicable as a highly sensitive marker for myoepithelioma.
