急性冠脉综合征患者血清Gal-3浓度与病变程度及肌钙蛋白的相关性

Correlation of Serum Gal-3 Concentration with Disease Degree and Troponin in Patients with Acute Coronary Syndrome

摘要 目的探讨急性冠脉综合征(ACS)患者血清半乳糖凝集素3(Gal-3)与冠状动脉病变程度、肌钙蛋白的相关性,为尽早判断ACS的危险程度,并给予相应治疗提供一定的实验室依据。方法收集2018年9月~2020年6月于北华大学住院行冠脉造影确诊的ACS患者60例,收集24例行冠脉造影正常的非ACS患者作为对照组;入选病例均采集一般临床资料,均检测血清Gal-3及血清cTnⅠ,分析ACS患者血清Gal-3与冠脉病变程度及肌钙蛋白的相关性。结果ACS组血清Gal-3水平高于对照组(P<0.01);吸烟率、体重指数(BMI)、糖尿病患病率、高血压患病率、血肌酐水平高于对照组,左心室射血分数低于对照组(P<0.05);血清Gal-3水平随病变血管数的增加而增加,不同病变血管数比较,差异有统计学意义(P<0.01);cTnⅠ+患者血清Gal-3水平高于cTnⅠ-患者[16.85(11.46,20.62)vs 7.73(4.89,10.45),P<0.01]。在cTnⅠ+患者中,cTnⅠ浓度与血清Gal-3水平呈正相关(r=0.910,P<0.01)。结论ACS患者血清中Gal-3与病变严重程度及cTnⅠ浓度呈正相关,可以结合二者水平,作为筛选高危患者的指标之一。 Objective This study intends to investigate the relationship between serum galectin-3(Gal-3)and the degree of coronary artery disease and troponin in patients with acute coronary syndrome(ACS),in order to determine the risk of ACS as soon as possible,and provide certain laboratory basis for corresponding treatment.Methods A total of 60 patients with ACS who were hospitalized at Beihua University and received coronary angiography from September 2018 to June 2020 were collected.24 non-ACS patients with normal coronary angiography were collected as the control group;general clinical data were collected from the selected cases,Both serum Gal-3 and serum cTnⅠwere detected,and the correlation between serum Gal-3 and coronary artery disease degree and troponin in ACS patients was analyzed.Results Serum Gal-3 levels in the ACS group were higher than those in the control group(P<0.01);smoking rate,body mass index(BMI),prevalence of diabetes,prevalence of hypertension,blood creatinine levels were higher than those in the control group,the left ventricular ejection fraction was lower than that of the control group(P<0.05);The level of serum Gal-3 increased with the increase of the number of diseased blood vessels,the difference of the number of different diseased blood vessels was statistically significant(P<0.01);Serum Gal-3 levels of cTnⅠ+patients were higher than those of cTnⅠ-patients[16.85(11.46,20.62)vs 7.73(4.89,10.45),P<0.01].In cTnI+patients,the concentration of cTnI was positively correlated with serum Gal-3 level(r=0.910,P<0.01).Conclusion Serum Gal-3 in ACS patients is positively correlated with the severity of the lesion and the concentration of cTnI.The two levels can be combined as one of the indicators for screening high-risk patients.
