直接前入路全髋关节置换术治疗大骨节病性髋关节炎的临床效果

Clinical effect of direct anterior arthroplasty total hip arthroplasty in the treatment of osteoarthritis after Kaschin-Beck disease

目的观察直接前入路全髋关节置换术(DAA-THA)治疗大骨节病(KBD)性髋关节炎的临床效果。方法回顾性分析2015年8月至2019年10月的21例KBD性髋关节炎行全髋关节置换术患者的临床资料,记录患者的手术相关情况和术后并发症发生情况,评价患者术前、术后1周及随访1、6个月的Harris、WOMAC、VAS评分。结果所有患者的手术均顺利完成,术后无严重并发症,随访6个月无假体松动、下沉及脱位等现象。患者术后6个月的Harris、WOMAC、VAS评分与术前存在显著差异(P<0.05)。结论DAA-THA治疗KBD性髋关节炎的损伤小,手术时间短,髋关节功能恢复快,术后效果好。

Objective To observe the clinical effect of direct anterior arthroplasty total hip arthroplasty(DAA-THA)in the treatment of osteoarthritis after Kaschin-Beck disease(KBD).Methods The clinical data of 21 patients with osteoarthritis after KBD undergoing total hip arthroplasty from August 2015 to October 2019 were retrospectively analyzed.The operation related conditions and postoperative complications of patients were recorded.Harris,WOMAC,and VAS scores were evaluated before operation,1 week after operation and 1 and 6 months after follow-up.Results The operations of all patients were successfully completed without serious postoperative complications,and there was no prosthesis loosening,subsidence or dislocation during the 6-month follow-up.The Harris,WOMAC and VAS scores of patients at 6 months after operation were significantly different from those before operation(P<0.05).Conclusion DAA-THA has the advantages of small injury,short operation time,quick recovery of hip joint function and good postoperative effect in the treatment of osteoarthritis after KBD.
