Neumann型增殖型天疱疮7例临床分析

Clinical Analysis of 7 Cases of the Neumann Type of Pemphigus Vegetans

目的分析和总结Neumann型增殖型天疱疮的临床、病理、诊断及治疗特点。方法回顾性分析2012年1月-2020年1月新疆自治区人民医院诊治的7例Neumann型增殖型天疱疮的临床资料。结果7例患者腋窝、小腿内侧、腹股沟区、肛门等皱褶处出现肥厚性暗褐色斑块。皮损组织病理检查示:表皮显著增生,局灶性海绵水肿,表皮内有嗜酸性粒细胞脓肿;直接免疫荧光(DIF):棘细胞间IgG均阳性。结论Neumann型增殖型天疱疮较为少见,糖皮质激素仍是治疗的首选药物。

Objective To analyze and summarize the clinical,pathological,diagnostic and therapeutic features of the Neumann type of pemphigus vegetans.Methods Seven cases of the Neumann type of pemphigus vegetans which were diagnosed and treated in Pepole's Hospital of Xinjiang Uygur Autonomous Region from January 2012 to January 2020 were retrospectively studied by reviewing the clinical data.Results Dark brown hypertrophic plaques were found in the flexures of the body like axilla,medial calf,inguinal region and anus in 7 patients.Histopathologic examination of the skin showed marked epidermal hyperplasia,focal sponge edema,eosinophilic abscess in the epidermis.Direct immunofluorescence(DIF)showed positive for interspinous IgG.Conclusion The Neumann type of pemphigus vegetans is rare,and corticosteroids are still the first choice for treatment.
