中西医结合治疗新型冠状病毒肺炎38例回顾性分析

Retrospective Analysis of 38 Cases with Coronavirus Disease 2019 Treated by Integrated Traditional Chinese and Western Medicine

目的通过回顾性分析探讨中西医结合治疗新型冠状病毒肺炎的临床疗效。方法收集武汉雷神山医院2020年2月16日至3月25日出院的64例新型冠状病毒肺炎患者临床资料,对其基本信息、中医证候、实验室检查、治疗方法等进行回顾性分析。按照干预方法的不同分为中西药组38例和西药组26例,西药组给予抗病毒、抗感染、辅助支持药物,中西药组在西药组治疗的基础上辨证论治给予中药汤剂及中成药治疗,统计并比较两组患者的临床症状消失时间、中医证候量表评分、体温复常时间、肺部CT影像好转时间,普通型转重型、死亡及不良反应发生情况,出院时实验室指标恢复正常情况。结果中西药组降钙素原复常率为100%,临床症状消失时间轻型(5.26±1.89)天、普通型(9.67±1.96)天、重型(12.12±1.43)天、恢复期(5.97±2.01)天,肺部CT影像好转时间(10.64±1.98)天,中医证候量表评分(6.84±3.78)分,体温复常时间(3.12±1.53)天,西药组降钙素原复常率为73.3%,临床症状消失时间轻型(7.18±2.15)天、普通型(12.71±1.38)天、重型(19.34±2.07)天、恢复期(7.83±1.52)天,肺部CT影像好转时间(14.53±2.07)天,中医证候量表评分(13.32±5.16)分,体温复常时间(5.26±1.72)天,中西药组各项指标均优于西药组(P<0.05);两组各发生1例不良反应,中西药组普通型转重型例数(0/38例)明显少于西药组(3/26例)(P<0.05)。结论中西医结合治疗新型冠状病毒肺炎能显著减轻患者的临床症状、缩短体温复常时间和肺部CT影像好转时间,其疗效优于单纯西药治疗。

Objective To explore the clinical efficacy of integrated Chinese and western medicine in the treatment of coronavirus disease 2019(COVID-19)by retrospective analysis.Methods Clinical data of 64 COVID-19 patients in Raytheon Hill Hospital from February 16 to March 25,2020 was collected including general information,traditional Chinese medicine(TCM)syndrome,laboratory examination and treatment methods for retrospective study.According to different intervention methods,they were divided into an integrated Chinese and western medicine group(38 cases)and a western medicine group(26 cases).The western medicine group was given antiviral,anti infective and auxiliary supporting drugs.The integrated treatment group was additionally given traditional Chinese medicine decoction and Chinese patent medicine according to syndrome differentiation and treatment.The time of clinical symptoms disappearance,TCM Syndrome Scale score,time to normal body temperature,lung CT improvement days,cases of common type patients turning to heavy or dying,adverse reactions,and laboratory indicators returning to normal when discharged were compared between groups.Results In the integrated treatment group,procalcitonin recovery rate was 100%,clinical symptoms disappear time of mild type(5.26±1.89)days,common type(9.67±1.96)days,severe type(12.12±1.43)days,recovery period(5.97±2.01)days,time for lung CT image improvement(10.64±1.98)days,TCM syndrome scale score(6.84±3.78)points,time for body temperature return to normal(3.12±1.53)days.While in the western medicince group,procalcitonin recovery rate was 73.3%,clinical symptoms disappear time of mild type(7.18±2.15)days,common type(12.71±1.38)days,severe type(19.34±2.07)days,recovery period(7.83±1.52)days,time for lung CT image improvement(14.53±2.07)days,TCM syndrome scale score(13.32±5.16)points,time for body temperature return to normal(5.26±1.72)days,the integrated treatment was supperior to the western medicine group significantly(P<0.05);one case of adverse reaction occurred in each of the 2 groups.The number of cases of common type turing to severe(0/38 cases)in the integrated treatment group was significantly less than that in the western medicine group(3/26 cases,P<0.05).Conclusion Integrated traditional Chinese and western medicine can significantly reduce the clinical symptoms,shorten the time of temperature normalization and the CT absorption time of lung in the treatment of COVID-19,the efficacy is better than that of western medicine alone.
