抗苗勒氏管激素在评估卵巢储备功能下降的临床价值

Clinical Value of Anti--Mullerian Hormone in Evaluating the Decline of Ovarian Reserve Function

目的:探讨抗苗勒氏管激素(anti-mullerian hormone,AMH)在评估卵巢储备功能下降(decreased ovarian reserve,DOR)的临床价值。方法:选取60例阴道B超诊断为DOR的患者作为研究组,另选取60例阴道B超诊断为卵巢储备功能正常的同期健康体检的女性作为对照组,检测120人的血清基础促卵泡生成素(follicle stimulating hormone,FSH)、促黄体生成素(luteinizing hormone,LH)、雌二醇(estradiol,E2)、AMH水平,比较各指标的测定结果及与阴道B超测定结果的相符率。结果:研究组血清基础E2、AMH水平低于对照组,组间比较,差异有统计学意义(P<0.05);研究组血清基础FSH水平高于对照组,组间比较,差异有统计学意义(P<0.05);AMH测定结果与阴道B超测定结果相符率最高。结论:AMH在评估DOR方面有一定优势,与阴道B超测定结果相符率高,可作为临床评估卵巢储备功能的重要观察指标。

Objective:To explore the clinical value of anti-mullerian hormone(AMH)in evaluating the decline of ovarian reserve function(DOR).Methods:60 patients with DOR diagnosed by B-mode ultrasound were selected as the study group,and 60 women with normal ovarian reserve function diagnosed by B-mode ultrasound as the control group.The serum levels of basic FSH,LH,E2 and AMH were measured in 120 patients.The results of each index and coincidence rate were compared with those of B-mode ultrasound.Results:The level of serum basic E2 and AMH in the study group was lower than that in the control group,and the difference between the two groups was statistically significant(P<0.05).The level of serum basic FSH in the study group was higher than that in the control group,and the difference between the two groups was statistically significant(P.c0.05).The results of AMH measurement and vaginal B-ultrasound accorded the highest.Conclusion:AMH has certain advantages in assessing DOR,and has a high agreement with the results of vaginal B-ultrasound.It can be used as an important observation index for clinical evaluation of ovarian reserve function.
