外科快速康复理念下术前是否采用机械性肠道准备对胃癌患者术后康复的影响

Effect of Mechanical Intestinal Preparation Before Surgery Under the Concept of Rapid Surgical Rehabilitation on the Postoperative Rehabilitation of Gastric Cancer Patients

目的:探讨外科快速康复(FTS)理念下术前是否采用机械性肠道准备(MBP)对胃癌患者术后康复的影响。方法:选取2017年8月-2019年8月于郑州市第三人民医院普外科行手术治疗的160例胃癌患者作为研究对象,按随机数表法分为观察组和对照组,每组各80例。对照组术前采用MBP,观察组术前不采用MBP,比较两组患者术后胃肠道功能恢复、术后并发症、首次下床活动时间、住院时间、术后体温及护理满意度情况。结果:观察组患者术后出现电解质失衡的发生率明显低于对照组(P<0.05);观察组的护理满意度较对照组高(P<0.05);在胃肠道功能恢复情况,术后出现感染、出血、吻合口瘘、胃瘫等并发症及术后首次下床活动时间、住院时间及术后体温上,两组结果比较差异均无统计学意义(P>0.05)。结论:外科快速康复理念下术前不采用机械性肠道准备有利于降低患者术后出现电解质失衡的发生率,提高患者的主观依从性及护理满意度,可安全应用于胃癌患者的术前准备中。

Objective:To explore the effect of using mechanical bowel preparation(MBP)before surgery on the concept of rapid surgical rehabilitation(FTS)on postoperative rehabilitation of gastric cancer patients.Methods:160 patients with gastric cancer who underwent general surgery in the hospital from August,2017 to August,2019 were selected as the research object.They were divided into an observation group and control group,with 80 cases in each group according to the random number table method.MBP was used before operation,and MBP was not used in the observation group before operation.The gastrointestinal function recovery,postoperative complications,time to first out of bed activity,length of hospital stay,postoperative temperature and nursing satisfaction of the two groups were compared.Results:The incidence of electrolyte imbalance in the observation group was significantly lower than that in the control group(P<0.05);the nursing satisfaction of the observation group was higher than that of the control group(P<0.05).Complications,such as infection,hemorrhage,anastomotic leakage,gastroparesis,postoperative time to get out of bed for the first time,length of hospital stay and postoperative temperature,were not statistically significant between the two groups(P>0.05).Conclusion:Without the use of mechanical bowel preparation before surgery under the concept of rapid surgical rehabilitation is helpful to reduce the incidence of postoperative electrolyte imbalance,improve the patients'subjective compliance and nursing satisfaction.It can be safely applied to the preoperative preparation of patients with gastric cancer.
