摘要 目的利用Meta分析探讨微小核糖核酸(microRNA)在子宫内膜癌中的诊断价值。方法采用计算机检索PubMed、Cochrane Library、Embase、万方、知网、维普等数据库,收集有关microRNA诊断子宫内膜癌的文献。对文献进行筛选及质量评价、数据提取,使用诊断准确性研究的质量评价工具(QUADAS-2)评估文献质量。使用统计软件对灵敏度和特异度进行合并,分析诊断比值比(DOR)、阳性似然比(PLR)、阴性似然比(NLR),绘制综合受试者工作曲线(SROC)及计算曲线下面积(AUC),利用Deeks′漏斗图评估发表偏倚。结果该研究共纳入6篇文献,其中子宫内膜癌患者215例,对照者171例。合并后的灵敏度为0.78(95%CI:0.74~0.82),特异度为0.77(95%CI:0.71~0.82)。该研究中PLR和NLR分别为3.4(95%CI:2.7~4.2)和0.29(95%CI:0.24~0.34),DOR为12.0(95%CI:9.0~16.0),AUC为0.84(95%CI:0.81~0.87)。Deeks′漏斗图是对称的,说明不存在发表偏倚。结论microRNA是一种有潜力的诊断子宫内膜癌的生物标志物,具有较高的诊断准确性。 Objective To use the Meta analysis to study the value of microRNA in the diagnosis of endometrial cancer(EC).Methods The literatures on microRNA for diagnosing EC were retrieved from the databases of PubMed,Cochrane Library,Embase,Chinese CNKI,Wan Fang,VIP,etc.,by computer.The literature screening,quality evaluation and data extraction were performed.The literature quality was evaluated by using the QUADAS-2 tool.The statistic software was used to pool the sensitivity and specificity,analyze the diagnostic odds ratio(DOR),positive likelihood ratio(PLR),negative likelihood ratio(NLR),summary receiver characteristic(SROC)curves,and the area under curve(AUC)were analyzed.Finally,the publication bias of included studies was assessed by using the Deeks′funnel plot.Results Six articles were included,including 215 cases of EC and 171 controls.The pooled sensitivity was 0.78(95%CI:0.74-0.82),and the specificity was 0.77(95%CI:0:71-0.82).PLR and NLR were 3.4(95%CI:2.7-4.2)and 0.29(95%CI:0.24-0.34),respectively.The DOR was 12.0(95%CI:9.0-16.0),with an area under the SROC curve of 0.84(95%CI:0.81-0.87).The Deeks′funnel plot was symmetric,indicating that the publication bias did not exist.Conclusion microRNA is a potential biomarker for the diagnosis of EC,and has the high diagnostic accuracy.
