祛疣汤联合光动力治疗对湿热血瘀型顽固性跖疣患者皮损程度、生活质量及复发情况的影响

Effects of Quyou Decoction Combined with Photodynamic Therapy on the Degree of Skin Lesion,Quality of Life and Recurrence in Patients with Intractable Plantar Warts of Damp-Heat and Blood Stasis Type

目的:探讨祛疣汤联合光动力治疗对湿热血瘀型顽固性跖疣患者皮损程度、生活质量及复发情况的影响。方法:将2018年3月至2019年6月河北中石油中心医院和廊坊市人民医院皮肤科接诊的100例湿热血瘀型顽固性跖疣患者按照随机数字表法分为对照组和观察组,每组50例。对照组患者给予光动力治疗,并外用维A酸乳膏;观察组患者给予光动力治疗联合祛疣汤浸泡治疗。观察两组患者的临床疗效、皮损改善情况、生活质量、T细胞亚群及复发情况。结果:观察组患者的总有效率为92.00%(46/50),较对照组的74.00%(37/50)明显更高,差异有统计学意义(P<0.05);治疗后,两组患者皮损各项评分及皮肤病生活质量指数总分与治疗前相比明显降低,且观察组患者与对照组相比明显更低,差异均有统计学意义(P<0.05)。治疗后,两组患者CD3+、CD4^+及CD4^+/CD8^+水平与治疗前相比明显升高,观察组患者与对照组相比明显更高,差异均有统计学意义(P<0.05);而两组患者CD8+水平与治疗前比较的差异均无统计学意义(P>0.05)。观察组患者的复发率为4.00%(2/50),较对照组的18.00%(9/50)明显更低,差异有统计学意义(P<0.05)。结论:对湿热血瘀型顽固性跖疣患者实施祛疣汤联合光动力治疗,其疗效优于光动力联合维A酸乳膏治疗,可有效促进患者皮损消退,明显改善患者的生活质量和细胞免疫功能,且能够显著降低复发的风险。

OBJECTIVE:To probe into the effect of Quyou decoction combined with photodynamic therapy on the degree of skin lesion,quality of life and recurrence in patients with intractable plantar warts of damp-heat and blood stasis type.METHODS:Totally 100 patients with intractable plantar warts of damp-heat and blood stasis type admitted into Hebei Petro China Central Hospital and Langfang People’s Hospital from Mar.2018 to Jun.2019 were selected and divided into control group and observation group via random number table,with 50 cases in each group.The control group was given photodynamic therapy combined with tretinoin cream for external use;while the observation group was given photodynamic therapy combined Quyou decoction for soaking.The clinical efficacy,improvements of skin lesion,quality of life,T cell subset and recurrence of both groups were observed.RESULTS:The total effective rate of observation group was 92.00%(46/50),which was significantly higher than that of the control group(74.00%,37/50),with statistically significant difference(P<0.05);after treatment,the various scores of skin lesion and the total scores of QOL indicator of the two groups were significantly lower than those of before treatment,and those of the observation group were significantly lower than the control group,with statistically significant differences(P<0.05).After treatment,the CD3^+,CD4^+and CD4^+/CD8^+levels of both groups were significantly higher than those of before treatment,and those of the observation group were significantly higher than the control group,with statistically significant differences(P<0.05);while there was no statistical significance in difference in CD8+levels between two groups before and after treatment(P>0.05).The recurrence rate of observation group was 4.00%(2/50),which was significantly lower than that of the control group(18.00%,9/50),with statistically significant difference(P<0.05).CONCLUSIONS:For patients with intractable plantar warts of damp-heat and blood stasis type,Quyou decoction combined with photodynamic therapy is more effective than photodynamic therapy combined with tretinoin cream,which can effectively promote the regression of skin lesions,significantly improve their quality of life and cellular immune function,and significantly reduce the risk of recurrence.
