渐进式心脏运动康复护理干预对经皮冠状动脉介入治疗后心功能的影响

Effect of Progressive Cardiac Exercise Rehabilitation Nursing Intervention on Cardiac Function after Percutaneous Coronary Intervention

目的探讨冠心病患者经皮冠状动脉介入术(PCI)早期进行渐进式心脏运动康复护理干预的价值。方法选择2017—2018年营口市中心医院收治的冠心病并行PCI患者120例,随机分为对照组和干预组各60例。对照组采用常规护理模式,干预组在对照组基础上,术后进行6个月的渐进式心脏运动康复护理干预,对比手术前后心功能改善情况。结果两组患者干预后射血分数(LVEF)均显著提高,与同组干预前相比LVEF差异有统计学意义(P<0.05);组间相比LVEF差异也有统计学意义(P<0.05)。当术前LVEF≤50%时,两组干预后LVEF均显著增加,与同组干预前相比差异有统计学意义(P<0.05);而组间差异无统计学意义(P>0.05)。当术前LVEF在50%~60%时,与同组干预前相比LVEF差异有统计学意义(P<0.05);组间相比LVEF差异也有统计学意义(P<0.05)。当术前LVEF≥60%时,无论是组内还是组间LVEF变化均不明显,差异均无统计学意义(P>0.05)。结论渐进式心脏运动康复护理干预能够改善冠心病行PCI患者术后的心脏泵血能力,并且这一效果与术前LVEF具有一定关系。

Objective To explore the value of early progressive cardiac exercise rehabilitation nursing intervention in patients with coronary heart disease undergoing percutaneous coronary intervention(PCI).Methods A total of 120 patients with coronary heart disease underwent PCI in Yingkou Central Hospital from 2017 to 2018 were randomly divided into control group and intervention group,with 60 cases in each group.The control group was given routine nursing mode,and the intervention group was given progressive cardiac exercise rehabilitation nursing intervention for 6 months after operation on the basis of the control group,The improvement of cardiac function before and after operation was compared.Results After intervention,the ejection fraction(LVEF)of the two groups were significantly improved.The difference was significant between before and after the intervention in the same group(P<0.05),and the difference of LVEF between the two groups was also significant(P<0.05).When preoperative LVEF≤50%,LVEF of the two groups increased significantly after intervention(P<0.05),but there was no significant difference between the two groups(P>0.05).When the preoperative LVEF was 50%~60%,there was significant difference in LVEF between before and after intervention in inter group comparison(P<0.05),and the difference in inter groups was also significant(P<0.05).When LVEF≥60%,no significant difference was found in LVEF for inter and intra-group comparisons(P>0.05).Conclusion Progressive cardiac exercise rehabilitation nursing intervention can improve the cardiac pumping ability of patients with coronary heart disease after PCI,and this effect has a certain relationship with preoperative LVEF.
