经脐单孔腹腔镜子宫肌瘤剔除术治疗子宫肌瘤的效果及安全性分析

Efficacy and Safety of Transumbilical Single Port Laparoscopic Myomectomy in the Treatment of Uterine Myoma

目的探讨经脐单孔腹腔镜子宫肌瘤剔除术治疗子宫肌瘤的效果及安全性。方法选择2019年2月—2020年2月在抚顺市中心医院接受手术治疗的子宫肌瘤患者154例,依据随机数字表法分成观察组及对照组各77例。对照组行常规三孔腹腔镜子宫肌瘤剔除术,观察组行经脐单孔腹腔镜子宫肌瘤剔除术。比较两组疗效、手术指标、并发症发生情况以及术后即刻及术后1d视觉模拟评分。结果观察组治疗总有效率为100.00%,高于对照组的97.40%,但差异无统计学意义(P>0.05)。观察组手术时间、术中出血量、术后引流量及住院时间均优于对照组,差异有统计学意义(P<0.05)。两组术后1d视觉模拟评分均低于术后即刻,且观察组较对照组更低,差异有统计学意义(P<0.05)。观察组并发症总发生率为3.90%,低于对照组的12.99%,差异有统计学意义(P<0.05)。结论经脐单孔腹腔镜子宫肌瘤剔除术治疗子宫肌瘤安全有效,而且能够有效改善患者术后疼痛情况。

Objective To investigate the effect and safety of transumbilical single port laparoscopic myomectomy in the treatment of uterine myoma.Methods A total of 154 patients with uterine myoma in Fushun Central Hospital were selected from February 2019 to February 2020.The patients were divided into the observation group and the control group(77 cases in each group)according to the random number table method.The control group was given routine three port laparoscopic myomectomy,and the observation group was given transumbilical single port laparoscopic myomectomy.The effect,surgical indicators,complications,and visual simulation score(VAS)at immediate post-operation and 1d after the operation were compared between two groups.Results The total effective rate of the observation group was 100.00%,higher than 97.40%of the control group(P>0.05).The operative time,intraoperative blood loss,postoperative drainage volume and hospitalization time of the observation group were significantly better than those of the control group(P<0.05).The VAS scores at 1d after operation in the two groups were significantly lower than those of immediate post-operation and the observation group was significantly lower than the control group(P<0.05).The total incidence of complications in the observation group was 3.90%,significantly lower than 12.99%of the control group(P<0.05).Conclusion Transumbilical single port laparoscopic myomectomy is safe and effective in the treatment of uterine myoma,and can effectively improve the postoperative pain of patients.
