摘要
目的探讨2型糖尿病患者血清甲胎蛋白(AFP)水平与代谢相关指标的相关性。方法选取中国人民解放军联勤保障部队第九〇〇医院莆田医疗区2型糖尿病住院治疗患者200例患者作为糖尿病组,100例体检人群作为健康对照。测定研究对象的血清AFP水平以及空腹血糖(FBG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbAlc)、空腹C肽(C0)、餐后2h C肽(C2)、低密度脂蛋白(LDL)和高密度脂蛋白(HDL)水平,并研究AFP与上述指标的相关性。结果糖尿病组患者血清AFP、FBG、2hPG、LDL高于健康对照组,而糖尿病组患者血清HDL水平显著低于健康对照组(t=5.23,20.32,13.71,6.67,10.25,2.70,10.94,12.83;P<0.05)。采用Spearman相关分析显示血清AFP水平与HbAlc、FBG、2hPG正相关,C0负相关(r=0.28,0.17,0.11,-0.18;P<0.05)。多元逐步回归分析显示HbAlc和C0是影响血清AFP的主要因素(β=1.19,-0.88;P<0.05)。结论2型糖尿病患者血清AFP水平高于常人显示其肝癌风险较高,血糖水平控制不佳可能是导致这一风险的原因。
Objective To investigate the correlation between serum AFP level and metabolic indexes in patients with type 2 diabetes mellitus.Methods 200 patients with type 2 diabetes in Putian Medical District of The 900 th Hospital of the Joint Security Force of the Chinese People’s Liberation Army were selected as diabetes group,and 100 healthy people were selected as healthy control group.The levels of serum AFP,fasting blood glucose(FBG),2-hour postprandial blood glucose(2 hPG),glycosylated hemoglobin(HbAlc),fasting C-peptide(C0),2-hour postprandial C-peptide(C2),low-density lipoprotein(LDL)and high-density lipoprotein(HDL)were measured.Results The serum levels of AFP,FBG,2 hPG and LDL in diabetic patients were higher than those in control group,but HDL was lower than that in control group(t=5.23,20.32,13.71,6.67,10.25,2.70,10.94,12.83;P<0.05).Spearman correlation analysis showed that serum AFP level was positively correlated with HbAlc,FBG and 2 hPG,while C0 was negatively correlated(r=0.28,0.17,0.11,-0.18;P<0.05).Multiple stepwise regression analysis showed that HbAlc and C0 were the main factors affecting serum AFP(β=1.19,-0.88;P<0.05).Conclusion The serum AFP level in patients with type 2 diabetes is higher than that in normal people,which indicates that they have a higher risk of liver cancer.High level of HbAlc and low level of C0 may be the reasons for this risk.
