早孕期TgAb、铁蛋白水平在妊娠期糖尿病患者中的检测价值探析

Detection Value of TGAb and Ferritin in Early Pregnancy in Patients with Gestational Diabetes

目的探析早孕期抗甲状腺球蛋白抗体(TgAb)、铁蛋白水平在妊娠期糖尿病患者中的检测价值。方法将2019年2月—2020年3月邓州市人民医院收治的59例妊娠期糖尿病患者作为研究组,同期137名血糖正常孕妇作为对照组,对两组人群的临床资料进行回顾性分析。分析TgAb、铁蛋白与FPG、HOMA-IR相关性,采用接受者操作特性曲线(ROC曲线)评估TgAb及铁蛋白水平在早孕期对妊娠期糖尿病的诊断价值。结果研究组铁蛋白、FPG、HOMA-IR水平及TgAb阳性率均明显高于对照组(P<0.05)。Pearson相关性分析结果显示,TgAb及铁蛋白与HOMA-IR呈正相关(P<0.05),与FPG无明显相关性(P>0.05);血清TgAb及铁蛋白水平预测妊娠期糖尿病的ROC曲线下面积(AUC)均明显高于参考线(P<0.05),铁蛋白水平截断值为149.99μg/L;联合预测ROC曲线AUC明显高于血清TgAb及铁蛋白水平(P<0.05)。结论早孕期检测孕妇血清TgAb及铁蛋白水平可对妊娠期糖尿病有预测作用,临床应用价值较高。

Objective To explore the detection value of anti thyroglobulin antibody(TGAb)and ferritin in early pregnancy in patients with gestational diabetes mellitus.Methods A total of 59 cases of gestational diabetes mellitus in Dengzhou People’s Hospital from February 2019 to March 2020 were selected as the study group,and 137 pregnant women with normal blood glucose were selected as the control group.The correlation of TGAb and ferritin with FPG and HOMA-IR was analyzed.Receiver operating characteristic curve(ROC curve)was used to evaluate the diagnostic value of TGAb and ferritin in early pregnancy for gestational diabetes mellitus.Results The levels of ferritin,FPG,HOMA-IR and positive rate of TGAb in the study group were significantly higher than those in the control group(P<0.05).Pearson correlation analysis showed that TGAb and ferritin were positively correlated with HOMA-IR(P<0.05),but not with FPG(P>0.05);the area under ROC curve(AUC)of serum TGAb and ferritin in predicting gestational diabetes mellitus was significantly higher than that of reference line(P<0.05);the cut-off value of protein level was 149.99μg/L;AUC of combined prediction ROC curve was significantly higher than that of serum TGAb and ferritin(P<0.05).Conclusion Detection of serum TGAb and ferritin levels in early pregnancy can predict gestational diabetes mellitus,and has high clinical application value.
