中青年和老年冠心病患者LDL不同亚型与AIP的相关性分析 被引量:2

摘要 目的探讨中青年和老年冠心病(CAD)患者的低密度脂蛋白(LDL)不同亚型与血浆致动脉硬化指数(AIP)间的相关性。方法连续入选256例通过冠状动脉造影术诊断CAD的患者为本研究对象,根据年龄分为中青年组(n=150,<65岁)和老年组(n=106,≥65岁)。采用线性聚丙烯酰胺凝胶对LDL各亚型进行定量分析,并计算AIP。比较2组间相关临床指标的差异,Spearman秩相关分析LDL各亚型与AIP的相关性,并以AIP为因变量应用多重线性回归分析其影响因素。结果中青年组AIP高于老年组(P<0.05)。中青年组中等LDL及其亚型LDL3、LDL4水平高于老年组(P<0.05)。全部CAD患者及中青年组、老年组的大而轻LDL与AIP的相关性均强于中等LDL和小而密LDL。多重线性回归模型显示,大而轻LDL、LDL1亚型是AIP的影响因素(P<0.05)。结论LDL不同亚型与中青年和老年人群CAD患者的相关性不同,同时应关注大而轻LDL及其LDL1亚型对AIP值的影响。 Objective To investigate the correlation between different subtypes of low density lipoprotein(LDL)and atherosclerotic index of plasma(AIP)in young and middle-aged and elderly patients with coronary heart disease(CAD).Methods A total of 256 patients with CAD diagnosed by coronary angiography were selected and divided into young and middle-aged CAD group(n=150,<65 years old)and elderly CAD group(n=106,≥65 years old)according to their age.The LDL subtypes were quantified by linear polyacrylamide gel,and AIP was calculated.The differences of clinical indexes between the two groups were compared.Spearman rank correlation was used to analyze the correlation between each subtype of LDL and AIP.AIP was used as the dependent variable,and the multiple linear regression was used to analyze influencing factors.Results The level of AIP was significantly higher in young and middle-aged CAD group than that of the elderly CAD group(P<0.05).The levels of medium LDL and its subtypes LDL3 and LDL4 were significantly higher in young and middle-aged CAD group than those in elderly CAD group(P<0.05).The correlation between large and light LDL and AIP was stronger in all CAD patients than that in medium LDL and small and dense LDL.Multiple linear regression model showed that large and light LDL and LDL1 subtypes were influencing factors of AIP(P<0.05).Conclusion The different subtypes of LDL have different correlation with CAD patients in young and middle-aged people.We should pay attention to the influence of large and light LDL and LDL1 subtypes on AIP.
