右美托咪啶用于老年患者下肢骨科手术麻醉中对提高镇痛效果及稳定患者呼吸的影响研究

Effect of Dexmedetomidine on Improving Analgesic Effect and Stabilizing Respiration in Elderly Patients Undergoing Lower Limb Orthopedic Surgery

目的分析右美托咪啶用于老年患者下肢骨科手术麻醉中对提高镇痛效果及稳定患者呼吸的影响。方法选取临沂市中医院2019年3月至2020年3月收治的老年下肢骨科手术患者80例作为研究对象,采用随机数字表法分为对照组与试验组,每组40例。对照组患者采取丙泊酚进行麻醉,试验组采取右美托咪定进行麻醉,将两组患者麻醉效果进行比较。结果麻醉后30 min,试验组患者的平均动脉压、心率及呼吸频率均优于对照组,组间比较差异有统计学意义(P<0.05);麻醉后60 min,试验组患者的心率、平均动脉压及呼吸频率均好于对照组,组间比较差异有统计学意义(P<0.05);试验组患者用药后5 min、30 min、60 min的镇痛效果及麻醉效果满意率均明显好于对照组,差异有统计学意义(P<0.05)。结论右美托咪啶在老年患者下肢骨科手术麻醉中效果显著,可明显改善患者的呼吸循环系统,镇痛效果较佳,患者满意程度较高,具备推广和使用价值。

Objective To analyze the effect of dexmedetomidine on improving the analgesic effect and stabilizing the respiration of elderly patients in lower limb orthopedic surgery.Methods A total of 80 elderly patients with lower limb orthopedic surgery in Linyi Hospital of Traditional Chinese Medicine from March 2019 to March 2020 were selected as the research objects.They were randomly divided into control group and experimental group,40 cases in each group.The control group was anesthetized with propofol and the experimental group was anesthetized with dexmedetomidine.Results 30 min after anesthesia,the arterial pressure,heart rate and respiratory rate of patients in the experimental group were better than those in the control group,and the difference was statistically significant(P<0.05).After 60 min,the heart rate,arterial pressure and respiratory rate of patients in the experimental group were better than those in the control group,and the difference between the two groups was statistically significant(P<0.05);the analgesic effect and satisfaction rate of anesthesia effect in the experimental group were significantly better than those in the control group at 5 min,30 min and 60 min,and the difference was statistically significant(P<0.05).Conclusion The dexmedetomidine in elderly patients with lower limb orthopedic surgery anesthesia effect is significant,can significantly improve the patient's respiratory and circulatory system,analgesic effect is better,patient satisfaction is high,has the promotion and use value.
