Elucidation of Differentiation and Treatment of Coma in Defensive System Syndrome by ZHAO Shao-Qin

整理赵绍琴教授对于卫分昏迷及卫气分误用寒凉所致高热昏迷的治法及用药原则。卫分昏迷虽为出现高热昏迷的危急重症,但不可再擅用寒凉阻滞气机,治疗应以辛味宣通为主,根据寒凉凝滞的程度及郁阻原因分别选用不同的辛味药进行治疗,兼用栀子豉汤、升降散开其火郁,郁开气达而火泄;气分热炽误用寒凉而昏迷,看似属于营分,实则为寒邪所郁闭,亦不宜再使用咸寒的营分治疗法则,而应以辛味药金银花、连翘等开其郁闭,用石菖蒲、郁金急开心窍,兼顾病因,稍佐以温药破开寒闭,使热邪有出路。

The article introduces ZHAO Shao-Qin’s therapeutic principle and medication rules for the treatment of coma in defensive system(qifen)syndrome or high-fever coma in defensive system syndrome caused by misuse of cold herbs. Coma in defensive system syndrome is usually accompanied by the emergent and critical symptoms of high-fever coma, but the cold herbs should not be used again to avoid the blockage of qi movement. For the treatment of coma in defensive system syndrome, pungent-taste herbs can be mainly used for dispersing and dredging,and the use of pungent-taste herbs should be varied according to the degree of cold obstruction and the causes of stagnation. Meanwhile, Zhizi Chi Decoction and Shengjiang Powder can be used for helping the dispersion of the fire stagnation,ensuring the smooth qi movement and purging the fire. For the treatment of coma caused by misuse of cold herbs due to the excessive heat at defensive system,pungent-taste herbs such as Flos Lonicerae and Fructus Forsythiae can be used to disperse the stagnation,herbs of Rhizoma Acori Tatarinowii and Radix Curcumae can be used to induce resuscitation urgently and then warm-nature herbs can be chosen for breaking the cold blockage to let the heat pathogen out. Coma caused by misuse of cold herbs due to the excessive heat at defensive system seems to have the manifestations of nutrient system(yingfen) syndrome, and its pathogenesis is actually due to the blockage of cold pathogen,so the salty-cold herbs for nutrient system syndrome are prohibited.
