泌乳素对肉芽肿性小叶性乳腺炎的促炎作用研究 被引量:8

摘要 目的探讨肉芽肿性小叶性乳腺炎(GLM)与泌乳素之间的关系并分析泌乳素对GLM的促炎作用。方法纳入2016年6月至2018年3月在上海中医药大学附属龙华医院乳腺外科就诊并经病理检查明确诊断为GLM的患者64例及健康体检女性30例,对GLM患者与健康人群的泌乳素水平进行比较;观察GLM患者的乳头凹陷情况、乳头溢液情况与泌乳素水平间的关系;同时观察GLM患者泌乳素水平与患者血清炎症指标间的关系。结果GLM患者与健康体检者泌乳素水平比较,差异有统计学意义(W=1155,P<0.05)。GLM患者血清泌乳素水平与白细胞水平呈正相关(r=0.249,P<0.05),泌乳素与IL-6水平也呈正相关(r=0.274,P<0.05)。泌乳素水平与患者乳头凹陷无明显相关性(r=-0.159,P=0.211),泌乳素水平与患者乳头溢液无明显相关性(r=-0.028,P=0.825)。结论泌乳素对GLM有促炎作用。 Objective To discuss the relationship between the granulomatous lobular mastitis(GLM)and prolactin,and analyze the inflammation causing effect of prolactin on GLM.Methods A total of 64 patients diagnosed with GLM by definite pathological examination from Jun 2016 to March 2018 and 30 healthy people were enrolled.Prolactin level was compared between GLM patients and healthy people.The relationship between nipple depression,nipple discharge and prolactin level in GLM patients were observed.At the same time,the relationship between prolactin level and serum inflammatory markers in GLM patients was observed,and the inflammation causing effect of prolactin in GLM was analyzed.Results The difference of prolactin level between GLM patients and healthy people was statistically significant(W=1155,P<0.05).Serum prolactin level was positively correlated with leukocyte level in GLM patients(r=0.249,P<0.05),it is also positively correlated with IL-6 level(r=0.274,P<0.05).Correlation analysis showed that there is no statistical significance between prolactin and nipple depression(r=-0.159,P=0.211).Correlation analysis showed that there is no statistical significance between polactin and nipple discharge(r=-0.028,P=0.825).Conclusion Prolactin has a pro-inflammatory effect on GLM.
