周围神经松解术治疗糖尿病足的临床效果分析

Clinical Effect of Peripheral Neurolysis in the Treatment of Diabetic Foot

目的探讨周围神经松解术治疗糖尿病足的临床效果。方法随机选取2019年1月~2020年1月期间就诊于北华大学附属医院的52例糖尿病足患者作为研究对象,并对其实施周围神经松解术,比较术前、术后患者的足部感觉、两点辨别觉、疼痛程度、神经传导速度以及欧米诺试验情况。结果术后3个月患者足部感觉变化缓解率为98.08%,效果较好;术后患者的疼痛视觉模拟评分(VAS)与术前相比明显降低,差异有统计学意义(P<0.001);术后患者的两点辨别觉与术前相比明显降低,差异有统计学意义(P<0.001);术后患者的腓总神经和胫神经的感觉传导速度和运动传导速度均得到了明显的提升,差异均有统计学意义(P<0.001);术后欧米诺试验总有效率为95.08%,效果较好。结论应用周围神经松解术治疗糖尿病足可有效缓解患者疼痛、麻木等症状,同时可以促进神经功能的恢复,值得临床应用和推广。

Objective To discuss the clinical effect of peripheral neurolysis on diabetic foot.Methods 52 patients with diabetic foot treated in the Affiliated Hospital of Beihua University from January 2019 to January 2020 were enrolled.The patients were treated with peripheral neurolysis.The foot sensation,two-point discrimination sense,pain degree,nerve conduction,and neuropad test were examined before and after surgery.Results The remission rate of foot sensation was 98.07%3 months after operation.The score of visual analogue scales(VAS)after operation was significantly lower than that before operation(P<0.001).The two-point discrimination sense after operation was significantly lower than that before operation(P<0.001).After operation,the sensory conduction velocity and motor conduction velocity of the common peroneal nerve and tibial nerve were significantly increased compared with those before operation(P<0.001).The neuropad test showed that the overall response rate of operation was 95.08%.Conclusion The application of peripheral neurolysis in the treatment of diabetic foot might effectively relieve pain,numbness and other symptoms,and promote the recovery of nerve function.
