叙事护理在2型糖尿病患者门诊健康教育中的应用研究

Application of narrative nursing in outpatient health education for type 2 diabetes patients

目的探讨叙事护理在门诊健康教育中的运用效果。方法选择2017年3月至2019年3月在护理门诊就诊的155例2型糖尿病患者作为研究对象,根据接诊顺序随机交叉分为对照组(74例)和实验组(81例),实验组采取包含素材收集、编辑素材、叙事干预的叙事健康教育模式,对照组采用传统健康教育方法,比较两组患者健康教育前后的代谢指标、自我管理行为、焦虑和抑郁评分。结果健康教育后实验组患者空腹血糖、餐后2 h血糖、糖化血红蛋白、焦虑和抑郁评分低于对照组(P<0.01或P<0.05),患者自我管理行为能力高于对照组(P<0.01)。结论叙事护理健康教育模式能促进护患情感同盟的建立,提高糖尿病门诊患者主动参与自我行为管理的意识和能力,并达到有效控制血糖的目的。

Objective To explore the effect of narrative nursing in outpatient health education.Methods A total of 155 patients with type 2 diabetes who were in the nursing clinic from March 2017 to March 2019 were selected as the research subjects.They were randomly divided into the control group(74 cases)and the experimental group(81 cases)according to the order of admission.The experimental group adopted a narrative health education model including material collection and editing,and narrative intervention,while the control group adopted traditional health education methods.The lipid metabolism indexes,self-management behavior,anxiety and depression scores of the two groups of patients before and after health education were compared.Results The fasting blood glucose,2 h postprandial blood glucose,glycosylated hemoglobin index,anxiety and depression scores of the experimental group after health education were lower than those of the control group(P<0.01 or P<0.05),and the patients had higher self-management ability than the control group(P<0.01).Conclusion The narrative nursing health education model can promote the establishment of the nurse-patient emotional alliance,improve the awareness and ability of diabetic outpatients to actively participate in self-behavior management,and achieve the goal of effective blood glucose control.
