阿尔茨海默病患者血清miR-137的表达及其与预后的关系

Study on the expression of miR-137 in serum of patients with Alzheimer's disease and its relationship with prognosis

目的研究miR-137在阿尔茨海默病(AD)患者血清中的表达特点及其与预后的关系。方法选取2017年01月至2019年01月在作者医院收治的150例AD患者及150名同期体检的健康体检者为研究对象,通过实时荧光定量PCR(qRT-PCR)检测各组血清miR-137表达水平。比较AD组和对照组血清miR-137表达的差异,并分析AD患者血清miR-137表达与血清三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、同型半胱氨酸(Hcy)、脂联素(ANP)及血浆β-淀粉样蛋白(Aβ40、Aβ42)水平的相关性。根据患者生存情况判断患者预后,比较预后良好组和不良组血清miR-137水平差异。结果与对照组比较,AD组患者血清miR-137表达水平低(0.42±0.03比1.01±0.14;t=50.469,P<0.01),血清Hcy、LDL-C、Aβ42水平及Aβ42/Aβ40比值高(均P<0.01),ANP水平低(P<0.01);AD患者血清miR-137表达水平与LDL-C(r=-0.517,P<0.05)、Hcy(r=-0.523,P<0.05)、Aβ42(r=-0.508,P<0.05)水平及Aβ42/Aβ40比值(r=-0.454,P<0.05)呈负相关,与ANP水平(r=0.626,P<0.05)、简易精神状态量表(MMSE)评分(r=0.347,P<0.05)呈正相关;预后良好者m iR-137表达水平显著高于预后不良AD患者(0.49±0.05比0.31±0.02;t=20.256,P<0.05)。结论miR-137在AD患者血清中表达量降低,其表达水平与LDL-C、Hcy、ANP水平,MMSE评分及患者不良预后有一定相关性,可能成为AD血清标志物,在预测患者病情进展和不良预后发生方面可能具有一定意义。

Objective To study the expression of miR-137 in serum of patients with Alzheimer's disease(AD)and its relationship with prognosis.Methods 150 AD patients admitted to our hospital from January 2017 to January 2019 were selected(the AD group)and 150 healthy people who underwent health examination in the author's hospital at the same time were selected(the NC group)as the control subjects.Real-time fluorescence quantitative PCR(qRT-PCR)was used to detect the expression of miR-137 in serum of each group and we analyzed the differences between the two groups.The general clinical data of each group was collected to analyze the correlations with the miR-137 expression,which included the levels of serum triglyceride(TG),total cholesterol(TC),low density lipoprotein(LDL-C),high density lipoprotein(HDL-C),homocysteine(Hcy),adiponectin(ANP),Aβ40 and Aβ42.The prognosis of the experimental group was evaluated by the patient survival to judge the prognosis of patients and analyze the different expression of serum miR-137 between the good and poor prognosis groups.Results Compared with the NC group,the level of serum miR-137 expression in the AD group was lower(0.42±0.03 vs.1.01±0.14;t=50.469,P<0.01),the levels of Hcy,LDL-C,Aβ42 and Aβ42/Aβ40 in the AD group were higher(P<0.01),and the level of ANP was lower(P<0.01).The level of serum miR-137 in the AD patients was negatively correlated with LDL-C(r=-0.517,P<0.05),Hcy(r=-0.523,P<0.05)Aβ42(r=-0.508,P<0.05),Aβ42/Aβ40 levels(r=-0.454,P<0.05),and positively correlated with ANP level(r=-0.626,P<0.05)and Mini-mental State Examination(MMSE)score(r=0.347,P<0.05).The expression level of miR-137 in the patients with good prognosis was significantly higher than that in the patients with poor prognosis(0.49±0.05 vs.0.31±0.02;t=20.256,P<0.01).Conclusions The expression of miR-137 in serum of AD patients is decreased,which is related to the changes of LDL-C,Hcy,ANP level and MMSE score,and may be used as a serum marker for AD which predicts disease progression and prognosis.
