某皮肤专科门诊用药咨询情况分析

Analysis of medication consultation in an outpatient department of dermatology

目的通过开展专科门诊用药咨询服务,探讨药师在专科门诊治疗过程中发挥的作用,促进临床合理用药。方法依据《医疗机构药学服务规范》,采集我单位门诊2019年7—12月间专科门诊用药咨询记录及反馈资料,按照咨询人员基本情况、咨询药物种类、咨询问题及回答结果归类总结,调查咨询满意度,提供个体化的药学服务,并进行临床用药合理性指导分析。结果共收集门诊有效用药咨询677例,其中男33.38%(226例)、女66.62%(451例)。年龄26~78岁,平均(50.49±13.64),40岁以上咨询人数占83.16%(563例)。患者及患者家属咨询人次占较大比例,占90.55%(613例)。咨询药物种类以抗组胺药比例最多,占34.71%(235)。患者及家属、公众咨询内容以用法用量比例最高,分别占37.22%(252例)、1.92%(13例),医护人员咨询内容主要为药物相互作用及特殊人群用药,分别占2.36%(16例)、1.18%(8例)。患者及家属、公众、医护人员咨询满意度分别为99.67%、92.11%、100%(χ2=28.16,P=0.005)。结论建立专科门诊用药咨询服务能充分体现药师专业价值,构建专科药学服务体系,提高用药合理性。

Objective To explore the role of pharmacists in the treatment process of specialist outpatient service through the development of special outpatient drug consultation service,so as to promote clinical rational drug use.Methods According to the"Pharmaceutical Service Standards for Medical Institutions",the medication consultation records and feedback data of the specialist outpatient service were collected from July to December 2019.According to the basic situation of the consultants,the types of consulted drugs,the consultation questions,and the answer results were classified and summarized,the consultation satisfaction was investigated,the individualized pharmaceutical services were provided,and the rationality of clinical medication was analyzed.Results A total of 677 effective medication consultations in the outpatient service were collected,including 33.38%(226 cases)males and 66.62%(451 cases)females.The average age was(50.49±13.64)years from 26 to 78 years old,and 83.16% of them(563 cases)were over 40 years old.The patients and their families accounted for 90.55%(613 person-times)of consultation.Antihistamines accounted for 34.71%(235)of consulting drugs.The usage and dosage of patients and their families,and the public consultation were the highest,accounting for 37.22%(252 cases)and 1.92%(13 cases)respectively.The consultation contents of medical staff mainly included drug interactions and medications for special populations,accounting for 2.36%(16 cases)and 1.18%(8 cases)respectively.The degrees of consultation satisfaction of the patients and their families,the public,and the medical staff were 99.67%,92.11%,and 100% respectively(χ^2=28.16,P=0.005).Conclusion The establishment of drug consultation service in special outpatient department can fully reflect the professional value of pharmacists,build a special pharmaceutical service system,and improve the rationality of medication.
