针灸加温经暖宫贴治疗肾虚宫寒型排卵障碍性不孕症临床观察

Clinical Observation on Acupuncture and Moxibustion Combined with Wenjing Nuangong Paste in the Treatment of Ovulatory Dysfunction Infertility of Kidney Deficiency and Cold Type

目的明确针灸配合"温经暖宫贴"贴敷治疗排卵障碍性不孕(肾虚宫寒型)的临床疗效;并进一步阐述了该方法起效与改善子宫内膜厚度、卵泡发育有关。方法纳入60例2018年1月1日—2018年12月31日收治的不孕症患者。SAS9.1程序随机分组,观察组采取针灸+温经暖宫贴治疗,对照组则采取口服克罗米芬治疗。治疗3个月经周期后,观察2组患者治疗前后子宫内膜厚度、卵泡发育情况、中医证候积分以及排卵率和妊娠率。结果观察组和对照组子宫内膜厚度分别为(8.67±1.42)mm、(7.38±1.22)mm,卵泡成熟数分别为(1.7±0.7)mm、(1.3±0.6)mm,差异均有统计学意义(P<0.05);观察组各中医证候积分均低于对照组且差异有统计学意义(P<0.05);观察组排卵率及妊娠率均高于对照组,差异有统计学意义(P<0.05)。结论在治疗肾虚宫寒型排卵障碍性不孕症时,应用针灸加上温经暖宫贴给予治疗,可有效促进卵泡发育,增加子宫内膜厚度,改善临床症状,对提高患者的排卵率、妊娠率疗效显著,值得在临床中推广使用。

Objective To clarify the clinical effect of acupuncture combined with Wenjing Nuangong paste in the treatment of anovulatory infertility(kidney deficiency and cold type),and to further expound the effect of this method is related to the improvement of endometrial thickness and follicular development.Methods 60 infertile patients were enrolled in our hospital from January 1,2018 to December 31,2018.With SAS 9.1 program,they were randomly divided into two groups.The observation group was treated with acupuncture and moxibustion combined with Wenjing Nuangong paste,while the control group was treated with oral clomiphene.After three menstrual cycles,the endometrial thickness,follicular development,TCM syndrome score,ovulation rate and pregnancy rate were observed before and after treatment.Results Endometrial thickness in the observation group and the control group was(8.67±1.42)mm,(7.38±1.22)mm,and follicular maturation was(1.7±0.7)mm and(1.3±0.6)mm,respectively,and there had statistically significant differences(P<0.05).The scores of TCM syndromes in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The ovulation rate and pregnancy rate in the observation group were significantly higher than those in the control group(P<0.05).Conclusion In the treatment of anovulatory infertility of cold type of kidney deficiency and uterus,acupuncture and moxibustion combined with Wenjing Nuangong paste can effectively promote the development of follicles,increase the thickness of endometrium and improve the clinical symptoms.It is effective to improve the ovulation rate and pregnancy rate of patients,and it is worth popularizing and using in clinic.
