滋阴降浊法对糖尿病肾病患者氧化应激及免疫功能的影响

The Influence of Method of Nourishing Yin and Reducing Turbidity on Oxidative Stress and Immune Function in Patients with Diabetic Nephropathy

目的:观察滋阴降浊法对糖尿病肾病患者氧化应激及免疫功能的影响。方法:将60例糖尿病肾病患者按照随机数字表法分为对照组和观察组,每组各30例。对照组给予常规西医治疗,观察组在对照组的基础上加用滋阴降浊汤治疗。比较两组患者治疗前后中医证候积分及肾功能指标[血尿素氮(blood urea nitrogen,BUN)、肌酐(serum creatinine,Scr)、24 h尿蛋白定量(urine protein quantification in 24 h,24 hUTP)、肾小球滤过率(glomerular filtration rate,GFR)、尿酸(uric acid,UA)]、氧化应激指标[超氧化物歧化酶(superoxide dismutase,SOD)、血清活性氧(reactive oxygen species,ROS)、晚期蛋白氧化产物(advanced protein oxidation products,AOPPS)]、炎症因子及免疫功能指标(CD4^+、CD8^+、CD4^+/CD8^+)变化情况。结果:两组患者治疗后中医证候积分、炎症因子水平低于本组治疗前,且观察组治疗后低于对照组治疗后,差异具有统计学意义(P<0.05)。两组患者治疗后GFR、SOD、CD4^+、CD4^+/CD8^+高于本组治疗前,BUN、SCr、24 hUTP、UA、ROS、AOPPS、CD8^+低于治疗前;观察组治疗后GFR、SOD、CD4^+、CD4^+/CD8^+高于对照组治疗后,BUN、SCr、24 hUTP、UA、ROS、AOPPS、CD8^+低于对照组治疗后,差异均有统计学意义(P<0.05)。结论:滋阴降浊法治疗糖尿病肾病,能够有效改善患者临床症状、氧化应激反应及免疫功能。

Objective:To observe the influence of method of nourishing yin and reducing turbidity on oxidative stress and immune function in patients with diabetic nephropathy.Methods:A total of 60 patients with diabetic nephropathy were randomly divided into the control group and the observation group,with 30 cases in each group.The control group received conventional Western medicine treatment,and the observation group received method of nourishing yin and reducing turbidity on the basis of the control group.The TCM syndrome score,renal function index,oxidative stress index,inflammatory factor and immune function index were compared between the two groups before and after the treatment.Results:After the treatment,TCM syndrome score and inflammatory factor levels of the two groups were lower than those of the same group before the treatment,and the above indexes of the observation group were lower than those of the control group after treatment,and all the differences were statistically significant(P<0.05).After the treatment,GFR,SOD,CD4^+,CD4^+/CD8^+in two groups were higher than those before the treatment,while BUN,SCR,24 hUTP,UA,ROS,AOPPS and CD8^+were lower than those before the treatment.GFR,SOD,CD4^+,CD4^+/CD8^+in the observation group were higher than those in the control group after the treatment,while BUN,SCR,24 hUTP,UA,ROS,AOPPS and CD8^+in the observation group were lower than those in the control group,and all the differences were statistically significant(P<0.05).Conclusion:In terms of diabetic nephropathy,method of nourishing yin and reducing turbidity,can effectively improve the clinical symptoms,oxidative stress response and immune function.
