阿立哌唑联合氯氮平治疗对肥胖精神分裂症患者代谢的影响

Effect of aripiprazole combined with clozapine on metabolism in obese patients with schizophrenia

目的探讨阿立哌唑联合氯氮平治疗对肥胖精神分裂症患者代谢的影响。方法选择2019年1~12月在我院诊断治疗的精神分裂症患者60例为研究对象,分为观察组与对照组各30例。对照组采用氯氮平单独治疗,观察组在此基础上加用阿立哌唑。比较两组治疗前后体重、腰围、BMI、血糖、血脂水平,两组治疗期间不良反应情况。结果(1)治疗后,观察组体重为(77.2±4.5)kg、腰围为(97.1±4.0)cm、BMI为(28.5±1.3)kg/m2显著低于对照组,差异有统计学意义(P<0.05)。(2)对照组FBG为(6.0±1.1)mmol/L、HbA1c为(6.5±1.3)%、LDL-C为(4.4±0.8)mmol/L、TG为(3.6±0.6)mmol/L、TC为(6.6±0.7)mmol/L显著高于治疗前,HDL-C为(1.2±0.4)mmol/L显著低于治疗前,差异有统计学意义(P<0.05)。治疗后观察组FBG为(5.4±1.0)mmol/L、HbA1c为(5.9±1.2)%、LDL-C为(4.0±0.7)mmol/L、TG为(2.9±0.5)mmol/L、TC为(5.8±0.6)mmol/L显著低于对照组,差异有统计学意义(P<0.05)。(3)两组不良反应比较,差异无统计学意义(P>0.05)。结论阿立哌唑联合氯氮平治疗对肥胖精神分裂症患者可有效控制患者体重,缓解代谢异常。

Objective To investigate the effect of aripiprazole combined with clozapine on metabolism in obese patients with schizophrenia.Methods A total of 60 patients with schizophrenia diagnosed and treated in our hospital from January to December 2019 were selected as research objects,and randomly divided into observation group(n=30)and control group(n=30).The control group was treated with clozapine,while the observation group was additionally treated with aripiprazole on the basis of the control group.The body weight,waist circumference,BMI,levels of blood glucose and blood lipid were compared between the two groups before and after treatment,and the adverse reactions during the treatment were compared between the two groups.Results(1)After treatment,body weight was(77.2±4.5)kg,waist circumference was(97.1±4.0)cm and BMI was(28.5±1.3)kg/m2 were significantly lower in the observation group than those in the control group,with statistically significant differences(P<0.05).(2)In the control group,FBG was(6.0±1.1)mmol/L,HbA1c was(6.5±1.3%),LDL-C was(4.4±0.8)mmol/L,TG was(3.6±0.6)mmol/L,TC was(6.6±0.7)mmol/L were significantly higher than those before treatment,and HDL-C(1.2±0.4)mmol/L were significantly lower than those before treatment,with statistically significant differences(P<0.05).After treatment,FBG was(5.4±1.0)mmol/L,HbA1c was(5.9±1.2)%,LDL-C was(4.0±0.7)mmol/L,TG was(2.9±0.5)mmol/L and TC was(5.8±0.6)mmol/L were significantly lower in the observation group than those in the control group,with statistically significant differences(P<0.05).(3)There was no statistically significant difference in adverse reactions between the two groups(P>0.05).Conclusion Aripiprazole combined with clozapine can effectively control body weight of and alleviate the abnormal metabolism in obese patients with schizophrenia.
