摘要
目的检测卵巢癌患者血清富亮氨酸α2糖蛋白1(LRG1)、血管紧张素2(ANG2)、骨桥蛋白(OPN)的表达情况,探讨LRG1、ANG2、OPN联合检测对卵巢癌早期诊断的价值。方法选取2018年7月至2020年5月在河北北方学院附属第二医院肿瘤科确诊并住院治疗的卵巢肿瘤患者80例为研究对象,其中经病理检查确诊为卵巢癌患者35例作为卵巢癌组,卵巢良性肿瘤患者45例作为对照组。采用酶联免疫吸附法(ELISA)检测血清LRG1、ANG2、OPN表达水平;分析LRG1、ANG2、OPN表达水平与卵巢癌临床病理特征之间的关系;并通过研究对象工作特征曲线(ROC)评估血清LRG1、ANG2、OPN表达水平对卵巢癌的诊断价值;对卵巢癌发生的相关影响因素采用Logistic回归分析。结果卵巢癌组血清LRG1、ANG2、OPN表达水平均显著高于对照组(P<0.05)。卵巢癌III^IV期患者LRG1、ANG2、OPN表达水平均显著高于卵巢癌I^II期患者(P<0.05);卵巢癌低分化患者LRG1、ANG2、OPN表达水平均显著高于卵巢癌中高分化患者(P<0.05)。ROC结果显示,血清LRG1、ANG2、OPN诊断卵巢癌的AUC分别为0.870、0.850、0.910,截断值分别为100.51ng/mL、103.18pg/mL、47.29ng/mL,此时对应灵敏度分别为68.6%、85.7%、85.7%,特异性分别为91.1%、77.8%、95.6%。血清LRG1、ANG2、OPN联合检测卵巢癌的AUC为0.973,此时对应灵敏度和特异性分别为94.3%和95.6%。Logistic回归分析显示:血清LRG1、ANG2、OPN高表达是影响卵巢癌发生的独立危险因素。结论LRG1、ANG2、OPN在卵巢癌患者血清中表达水平明显升高,且与卵巢癌的发生及发展密切相关,具有一定的诊断价值,提示血清LRG1、ANG2、OPN可作为卵巢癌早期诊断的潜在生物学指标。
Objective To detect the expressions of leucine-richα2 glycoprotein 1(LRG1),angiotensin 2(ANG2)and osteopontin(OPN)in patients with ovarian cancer,and to explore the value of combined detection of LRG1,ANG2 and OPN in the early diagnosis of ovarian cancer.Methods A total of 80 patients with ovarian tumor diagnosed and hospitalized in the Department of oncology,the Second Hospital affiliated to Hebei North University from July,2018 to May,2020 were selected as the research objects.Among them,35 patients with ovarian cancer confirmed by pathological examination were enrolled in the ovarian cancer group,and 45 patients with ovarian benign tumor were enrolled in the control group.The expression levels of LRG1,ANG2 and OPN in serum were detected by enzyme-linked immunosorbent assay(ELISA).The relationship between the expression levels of LRG1,ANG2 and OPN and the clinicopathological characteristics of ovarian cancer was analyzed.The diagnostic value of serum LRG1,ANG2,and OPN expression levels in ovarian cancer was then evaluated by receiver operating characteristic curve(ROC)and the related influencing factors of ovarian cancer were analyzed by logistic regression.Results The expression levels of LRG1,ANG2 and OPN in the ovarian cancer group were significantly higher than those in the control group(P<0.05).The expression levels of LRG1,ANG2 and OPN among stage III-IV patients were significantly higher than those in stage I-II patients(P<0.05).The expression levels of LRG1,ANG2 and OPN in poorly differentiated ovarian cancer patients were significantly higher than those in moderately and well differentiated ovarian cancer patients(P<0.05).ROC results showed that the AUC of serum LRG1,ANG2 and OPN for the diagnosis of ovarian cancer were 0.870,0.850 and 0.910,respectively,and the cut-off values were 100.51 ng/mL,103.18 pg/mL and 47.29 ng/mL,respectively,with the corresponding sensitivity of 68.6%,85.7%,85.7%,and the specificity of 91.1%,77.8%and 95.6%,respectively.The AUC of combined detection of LRG1,ANG2 and OPN was 0.973,and the corresponding sensitivity and specificity were 94.3%and 95.6%,respectively.Logistic regression analysis showed that the high expressions of LRG1,ANG2 and OPN were independent risk factors of ovarian cancer.Conclusion The expression levels of LRG1,ANG2 and OPN in serum of patients with ovarian cancer are significantly increased.They are closely related to the occurrence and development of ovarian cancer,and have certain diagnostic value.It is suggested that serum LRG1,ANG2 and OPN can be used as potential biological indicators for the early diagnosis of ovarian cancer.
