糖化血红蛋白变异指数对糖尿病大血管病变风险的评估价值 被引量:6

The Evaluation Value of Hemoglobin Glycation Indexfor the Risk of Diabetic Macroangiopathy

摘要 目的探讨T2DM患者糖化血红蛋白变异指数(HGI)与糖尿病大血管病变之间的关系。方法收集1747例患者的临床资料,计算HGI值,并按三分位数法分为低HGI、中HGI、高HGI三组,评估HGI水平与糖尿病大血管病变发生的关系。结果三组患者之间年龄、糖尿病病程、SBP、DBP、BMI、FPG、HbA1c、尿酸、TG、LDL-C、颈动脉粥样硬化、下肢动脉病变和总的大血管病变的检出率相比较,差异有统计学意义(P<0.05)。经二分类Logistic回归分析发现,HGI升高是2型糖尿病患者发生大血管病变的风险因素(OR=1.339,P<0.001);中HGI组、高HGI组发生大血管并发症的风险分别是低HGI组的1.54倍(OR=1.540,P=0.004)、2.036倍(OR=2.036,P<0.001)(OR=1.201,P=0.017)。结论HGI对于预测糖尿病大血管病变的发生有重要的价值。 Objective To explore the relationship between hemoglobin glycation index(HGI)and diabetic macroangiopathy in T2DM patients.Methods We collected 1747 patients′clinical data,then calculated HGI value.These patients were divided into three groups according to the HGI value:low HGI group,medium HGI group and high HGI group.We then evaluated the relationship between HGI level and the occurrence of diabetic macroangiopathy.Results There were significant differences among the three groups in ages,the course of diabetes,SBP,DBP,BMI,FPG,HbA1c,uric acid,TG,LDL-C,and the detection rate of carotid atherosclerosis,lower limb artery disease and diabetic macroangiopathy(P<0.05).Results Logistic regression analysis showed that high level of HGI was a risk factor for diabetic macroangiopathy in people with type 2 diabetes mellitus(OR=1.339,P<0.001).The risk of diabetic macroangiopathy in the middle HGI group and high HGI group was 1.540 times(OR=1.540,P=0.004)and 2.036 times(OR=2.036,P<0.001)higher than that in the low HGI group.Conclusion HGI has an important value for predicting the occurrence of diabetic macroangiopathy.
