PTX 3在重症肺炎患儿血清中的表达及其与症因子、预后的关系

The Expression of PTX 3 in Serum of Children with Severe Pneumoniaand Its Relationship with Inflammatory Factors and Prognosis

目的探讨正五聚蛋白3(PTX 3)在重症肺炎患儿血清中的表达,并进一步分析其与炎症因子、预后的关系。方法选取2019年1月至2020年2月期间我院收治的重症肺炎患儿126例,根据是否需要机械通气将重症肺炎患儿分为机械通气组(86例)和无机械通气组(40例),根据出院时转归分为生存组(102例)和死亡组(24例)。检测所有患儿的PTX 3、白介素-6(IL-6)、降钙素原(PCT)、超敏C反应蛋白(hs-CRP)、D-二聚体(D-D)的表达水平。多因素分析重症肺炎患儿预后不良的危险因素,分析重症肺炎患儿血清PTX 3与炎症因子的相关性,受试者工作特征(ROC)曲线分析生物标志物对重症肺炎患儿预后的评估价值。结果机械通气组的IL-6、PCT、hs-CRP、D-D、PTX 3水平均高于无机械通气组,差异有统计学意义(P<0.05),死亡组的IL-6、PCT、hs-CRP、D-D、PTX 3水平均高于生存组,差异有统计学意义(P<0.05),PCT、hs-CRP、D-D、PTX 3水平过高均是重症肺炎患儿预后不良的危险因素(P<0.05),重症肺炎患儿血清PTX 3与IL-6、PCT、hs-CRP、D-D均呈正相关(P<0.05),PCT、hs-CRP、PTX 3对重症肺炎患儿的预后评估价值较高,曲线下面积分别为0.827、0.735、0.839。结论PTX 3在重症肺炎患儿血清中呈高表达,且与患儿的炎症反应程度和预后密切相关,可用于临床辅助评估患儿的预后。

Objective To investigate the expression of pentraxin 3(PTX 3)in the serum of children with severe pneumonia,and to further analyze its relationship with inflammatory factors and prognosis.Methods 126 patients with severe pneumonia who were admitted to our hospital from January,2019 to February,2020 were selected for the study.According to whether mechanical ventilation was required,the patients were divided into the mechanical ventilation group(86 cases)and non-mechanical ventilation group(40 cases).According to the outcomes at discharge,the patients were divided into the survival group(102 cases)and death group(24 cases).The expression levels of PTX 3,interleukin-6(IL-6),procalcitonin(PCT),high-sensitivity C-reactive protein(hs-CRP),and D-dimer(D-D)in all children were measured.Multiple factors were used to analyze the risk factors of poor prognosis in children with severe pneumonia,and the correlation between serum PTX 3 and inflammatory factors in children with severe pneumonia were analyzed.The receiver operating characteristic(ROC)curve was used to investigate the values in the prognosis evaluation of these biomarkers in children with severe pneumonia.Results The levels of IL-6,PCT,hs-CRP,D-D and PTX 3 in the mechanical ventilation group were significantly higher than those of the non-mechanical ventilation group(P<0.05).The levels of IL-6,PCT,hs-CRP,D-D and PTX 3 in the death group were significantly higher than those of the survival group(P<0.05).High levels of PCT,hs-CRP,D-D and PTX 3 were all risk factors for poor prognosis in children with severe pneumonia(P<0.05).Serum PTX 3 in children with severe pneumonia was positively correlated with IL-6,PCT,hs-CRP,and D-D(P<0.05).PCT,hs-CRP and PTX 3 had a high value in the prognosis evaluation of children with severe pneumonia.The area under the curve were 0.827,0.735 and 0.839,respectively.Conclusion PTX 3 is highly expressed in the serum of children with severe pneumonia,and it is closely related to the degree of inflammatory response and prognosis of children.It can be used for the clinical auxiliary evaluation of children′s prognosis.
