睡眠障碍伴发焦虑抑郁的脑血流自动调节功能的分析 被引量:7

摘要 目的:探究睡眠障碍伴发焦虑、抑郁患者的脑血流自动调节功能。方法:选取我院2017年1月至2019年12月期间收治的65例睡眠障碍伴发焦虑、抑郁患者作为观察组,另选取70例同期检查健康对象作为健康对照组。对比分析两组研究对象的脑血流自动调节功能、焦虑和抑郁评分,并采用pearman分汉密尔顿焦虑量表(Hamilton RatingScale for Anxiety,HAMA)、汉密尔顿抑郁评定量表(Hamilton Depression Rating Scale,HAMD)评分与脑血流自动调节功能参数的相关性。结果:观察组患者HAMA评分、HAMD评分均显著高于对照组,二者比较具有统计学差异(P<0.05)。经血流分析仪曲线计算,观察组患者的左、右侧脑半球相位均显著低于对照组,二者比较具有统计学差异(P<0.05)。采用pearman分HAMA评分与脑血流自动调节功能参数的相关性,发现HAMA评分HAMD评分与观察组患者的双侧大脑半球相位差、增益均无显著相关性(P>0.05)。结论:睡眠障碍伴发焦虑、抑郁患者的脑血流自动调节功能显著降低,但其调节功能损伤程度与患者的焦虑、抑郁程度无显著相关,临床上无法采用HAMA量表、HAMD量表评价其脑血流自动调节功能的受损程度。 Objective:To explore the automatic regulation of cerebral blood flow in patients with sleep disorders accompanied by anxiety and depression.Methods:A total of 65 patients with sleep disorders accompanied by anxiety and depression admitted to our hospital from 2017 to December 2019 were selected as the observation group,and 70 healthy controls were made the control group.The cerebral blood flow autoregulation function,anxiety and depression scores of the two groups were compared and analyzed,and the correlation between the Pearman Hamilton Rating Scale for Anxiety(HAMA)score,Hamilton Depression Rating Scale(HAMD)score and the cerebral blood flow autoregulation function parameters was used.Results:The HAMA score and HAMD score of the observation group were significantly higher than those of the control group,and there was a statistical difference between the two(P<0.05).Calculated by the blood flow analyzer curve,the left and right hemisphere phases of the observation group were significantly lower than those of the control group,and there was a statistical difference between the two(P<0.05).Using the correlation between Pearman's HAMA score and the parameters of cerebral blood flow autoregulation,it was found that there was no significant correlation between the HAMA score and the HAMD score of the observation group's bilateral cerebral hemisphere phase difference and gain(P>0.05).Conclusion:The cerebral blood flow autoregulation function of patients with sleep disorders accompanied by anxiety and depression is significantly reduced,but the degree of impairment of its regulatory function is not significantly related to the patient's anxiety and depression.The HAMA scale and HAMD scale cannot be used to evaluate the brain in clinical practice.The degree of damage to the automatic regulation of blood flow.
