鼻咽癌颅底侵犯的CT与MRI征象对比分析

Comparative analysis of CT and MRI features of skull base invasion in nasopharyngeal carcinoma

目的:探讨CT及MRI在鼻咽癌颅底侵犯诊断中的应用价值。方法:回顾性分析经病理证实的52例鼻咽癌的CT及MRI图像,并以放疗后6个月的影像资料和临床资料为参考依据,比较CT与MRI在鼻咽癌颅底受侵诊断中的应用价值。结果:52例放疗6个月后定性诊断结果为颅底受侵41例,其中颅底骨质受侵39例,颅底肌肉受侵41例。52例中,初诊CT诊断颅底骨质侵犯阳性32例,MRI诊断阳性39例;CT诊断颅底肌肉侵犯阳性37例,MRI诊断阳性40例。2种检查方法对颅底骨质侵犯及肌肉侵犯的诊断效能比较差异均有统计学意义(均P<0.05)。结论:CT与MRI联合成像在鼻咽癌颅底侵犯诊断中有较大的应用价值。

Objective:To explore the value of combined CT and MRI scan of skull base invasion in nasopharyngeal carcinoma.Methods:The CT and MRI images of 52 patients with nasopharyngeal carcinoma confirmed by pathology were analyzed retrospectively.The CT and MRI findings were compared and analyzed.Based on the imaging data and clinical data 6 months after radiotherapy,the application value of CT and MRI combined imaging in skull base invasion of nasopharyngeal carcinoma was compared.Results:Among the 52 patients with nasopharyngeal carcinoma,41 cases were diagnosed as skull base invasion after radiotherapy for 6 months,including 39 cases of skull base bone invasion and 41 cases of skull base muscle invasion.Among 52 cases with nasopharyngeal carcinoma,32 cases of bone invasion were positive for first CT examination and 39 cases for first MRI examination,37 cases of muscle invasion were positive by first CT examination and 40 cases for first MRI examination.There were significant statistical significances in the detection rate of the two methods(all P<0.05).Conclusion:CT and MRI combined imaging is of great value in the diagnosis of skull base invasion of nasopharyngeal carcinoma.
