电子支气管镜刷检结合活检对痰菌阴性肺结核的诊断意义

摘要 为了探讨痰菌阴性肺结核经电子支气管镜刷检结合活检的诊断价值,本文选择痰菌阴性肺结核患者55例行活检、电子支气管镜刷检,对镜下表现进行分析,对比经电子支气管镜刷检、活检单独检查与联合检查的诊断结果。经电子支气管镜发现,55例痰菌阴性肺结核患者镜下表现以干酪坏死型(25.45%)、瘢痕狭窄型(20.00%)及溃疡型(23.64%)为主,其次为增殖型(16.36%)、炎性浸润型(14.55%)。电子支气管镜刷检阳性检出率与活检相比,差异没有统计学意义(P>0.05),电子支气管镜刷检结合活检阳性检出率(70.91%)均高于电子支气管镜刷检(34.55%)、活检单独检出率(41.82%),差异有统计学意义(P<0.05)。电子支气管镜刷检结合活检能够提高痰菌阴性肺结核患者阳性检出率,为临床制定后续治疗方案提供参考依据。 In order to explore the diagnostic value of electronic bronchoscopy and biopsy for sputum-negative pulmonary tuberculosis,55 cases of sputum-negative pulmonary tuberculosis patients are selected for biopsy and electronic bronchoscopy,and the microscopic findings are analyzed and compared with electronic bronchoscopy.According to electronic bronchoscopy,55 cases of sputum-negative pulmonary tuberculosis patients show caseous necrosis(25.45%),scar stenosis(20.00%)and ulcer(23.64%),follow by proliferative(16.36%),inflammatory infiltrating type(14.55%).Compared with biopsy,the positive detection rate of electronic bronchoscopy brushing is not statistically significant(P>0.05).The positive detection rate of electronic bronchoscopy brushing combined with biopsy(70.91%)is higher than that of electronic bronchoscopy brushing(34.55%),the detection rate of biopsy alone(41.82%),the difference is statistically significant(P<0.05).Electronic bronchoscopy combined with biopsy can increase the positive detection rate of sputum-negative pulmonary tuberculosis patients,and provide a reference for clinical follow-up treatment plans.
