摘要
目的通过对癌胚抗原(CEA)、糖类抗原125(CA125)、糖类抗原19-9(CA19-9)、甲胎蛋白(AFP)等4种血清肿瘤标志物进行联合检测,以探讨其在原发性肝癌早期诊断中的应用价值。方法选取2017年1月至2019年12月在郑州市第六人民医院住院的原发性肝癌患者120例、肝炎患者120例及同期健康体检者480例,均进行CEA、CA125、CA19-9、AFP等4种血清肿瘤标志物检测。结果原发性肝癌患者、肝炎患者、健康体检者血清CEA[(6.97±0.16)、(3.78±0.15)、(3.08±0.14)ng/mL]、CA125[(288.56±11.85)、(34.62±11.11)、(11.75±5.68)u/mL]、CA19-9[(94.56±1.69)、(25.45±1.38)、(19.87±1.24)u/mL]、AFP[(3558.86±118.52)、(10.54±4.32)、(3.54±1.08)ng/mL]总体比较差异均有统计学意义(F=571.600,P<0.001;F=717.448,P<0.001;F=32.395,P=0.001;F=2690.472,P<0.001)。原发性肝癌患者、肝炎患者、健康体检者血清CEA、CA125、CA19-9、AFP水平依次降低,两两比较差异均有统计学意义(P均<0.05)。结论CEA、CA125、CA19-9、AFP等4种血清肿瘤标志物联合检测对原发性肝癌的早期诊断有比较高的价值,可有效辅助临床进行诊断,具有重要的临床应用价值。
Objective To explore the application value of carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125),carbohydrate antigen 19-9(CA19-9)and alpha-fetoprotein(AFP)in the early diagnosis of primary hepatocellular carcinoma.Methods From January 2017 to December 2019 in Zhengzhou Sixth People’s Hospital,120 primary hepatocellular carcinoma patients,120 hepatitis patients and 480 healthy controls without liver disease who underwent physical examination during this period were selected,and CEA,CA125,CA19-9,AFP were detected and compared.Results There were statistical differences in the serum CEA[(6.97±0.16),(3.78±0.15),(3.08±0.14)ng/mL],CA125[(288.56±11.85),(34.62±11.11),(11.75±5.68)u/mL],CA19-9[(94.56±1.69),(25.45±1.38),(19.87±1.24)u/mL],AFP[(3558.86±118.52),(10.54±4.32),(3.54±1.08)ng/mL]levels among the primary hepatocellular carcinoma patients,the hepatitis patients and the healthy controls(F=571.600,P<0.001;F=717.448,P<0.001;F=32.395,P=0.001;F=2690.472,P<0.001);the serum CEA,CA125,CA19-9 and AFP levels were decreased in turn in the primary hepatocellular carcinoma patients,the hepatitis patients and the healthy controls(P<0.05).Conclusion The combined detection of serum CEA,CA125,CA19-9 and AFP has a high detection rate in the early diagnosis of primary hepatocellular carcinoma,which can effectively assist clinicians in the diagnosis,and has important clinical application value.
