四物消风散治疗血虚风燥型慢性荨麻疹的临床疗效及对血清白细胞介素17水平的影响

Clinical observation of Siwu Xiaofeng powder on chronic urticaria with blood deficiency and wind-dryness pattern and its effects on serum interleukin 17

目的观察四物消风散治疗血虚风燥型慢性荨麻疹(CU)的临床疗效以及其对血清白细胞介素17(IL-17)水平的影响。方法将59例血虚风燥型CU患者按照随机数字表法分为2组,对照组29例予西医常规治疗,治疗组30例在对照组基础上加用四物消风散治疗。2组均治疗4周后统计疗效,比较2组治疗前后CU病情活动度(包括风团数目、风团最大直径、瘙痒程度、持续时间及发作次数评分)变化情况,比较2组治疗前后血清IL-17水平变化情况。结果治疗组总有效率73. 3%(22/30),对照组总有效率41. 4%(12/29),治疗组疗效优于对照组(P <0. 05)。2组治疗后风团数目、风团最大直径、瘙痒程度、持续时间及发作次数评分与本组治疗前比较均明显降低(P <0. 05),且治疗后治疗组风团数目、瘙痒程度、持续时间及发作次数评分均低于对照组(P <0. 05)。2组治疗后血清IL-17水平与本组治疗前比较均明显降低(P <0. 05),且治疗组低于对照组(P <0. 05)。结论四物消风散治疗血虚风燥型CU临床疗效确切,可明显改善患者临床症状,控制CU病情活动,减少风团数目,缩小风团直径,减轻瘙痒程度,减少病情持续时间及发作次数,其作用机制可能与降低血清IL-17水平,抑制炎性反应有关。

Objective To observe the clinical effects of Siwu Xiaofeng powder on chronic urticaria(CU)with blood deficiency and wind-dryness pattern and its effects on serum interleukin 17(IL-17).Methods Fifty-nine CU patients with blood deficiency and wind-dryness pattern were divided according to the random number table into the control group(n=29)with conventional western medications and the treatment group(n=30)with Siwu Xiaofeng powder based on the treatment of the control group.The treatment course of both groups was 4 weeks.The clinical efficacy in the two groups was evaluated.And the scores of CU activities(including wheals number,the maximum diameter of wheals,itching degree,duration time,and episode times)and the levels of serum IL-17 were compared in the two group before and after the treatment.Results The total effective rate was 73.3%(22/30)in the treatment group while 41.4%(12/29)in the control group.So the clinical effect in the treatment group were higher than that of the control group(P<0.05).The scores of CU activities and the levels of serum IL-17 in the two groups after the treatment were obviously lower than those before the treatment(P<0.05).And the scores of CU activities and the level of serum IL-17 in the treatment group were lower than those of the control group after the treatment(P<0.05).Conclusion Siwu Xiaofeng powder on CU with blood deficiency and wind-dryness pattern has a definite therapeutic effect,which can improve the clinical symptoms,control CU activities,decrease wheals number,reduce wheals diameter,relieve itching degree,and shorten duration time and episode times.And its mechanism may be related to reducing the serum IL-17 level and inhibiting the inflammatory action.
