全髋关节置换术治疗老年股骨颈骨折的临床观察

Clinical observation of total hip arthroplasty on femoral neck fracture in the elderly

目的观察老年股骨颈骨折患者行全髋关节置换术治疗的临床效果。方法选取我院2018年7月~2019年7月收治的66例老年股骨颈骨折患者通过双盲随机法分为对照组与实验组,对照组33例患者行半髋关节置换术治疗,实验组33例患者行全髋关节置换术治疗,比较两组患者治疗效果。结果实验组患者其手术时间、术中出血量、术后引流时间、术后首次下床活动时间均优于对照组,差异有统计学意义(P<0.05)。实验组术后3个月、术后6个月假体髋臼前倾角、假体髋臼外展角较对照组患者对应时间点的假体髋臼前倾角、假体髋臼外展角小,差异有统计学意义(P<0.05)。实验组术后6个月恢复优良率(84.84%)比对照组高,差异有统计学意义(P<0.05)。实验组术后并发症发生率(6.06%)比对照组低,差异有统计学意义(P<0.05)。结论老年股骨颈骨折患者行全髋关节置换术可取得较半髋关节置换术更优的疗效。

Objective To observe the clinical efficacy of total hip arthroplasty on femoral neck fracture in the elderly.Methods A total of 66 elderly patients with femoral neck fracture admitted to our hospital from July 2018 to July 2019 were randomly divided into the control group(n=33)and the experimental group(n=33).The control group was given half hip arthroplasty,while the experimental group was given total hip arthroplasty.The therapeutic efficacies of the two groups were compared.Results The operation time,intraoperative blood loss,postoperative drainage time and the first ambulation time in the experimental group were better than those in the control group,the difference was statistically significant(P<0.05).The acetabular anteversion angle and acetabular abduction angle of the experimental group at 3 months and 6 months after operation were lower than those of the control group at the corresponding time points,the difference was statistically significant(P<0.05).The excellent and good rate of the experimental group(84.84%)was higher than that of the control group at 6 months after operation,the difference was statistically significant(P<0.05).The incidence of postoperative complications in the experimental group(6.06%)was lower than that in the control group(P<0.05).Conclusion The therapeutic efficacy of total hip arthroplasty in the elderly patients with femoral neck fracture is obviously superior to that of half hip arthroplasty.
