摘要
目的:探讨胸腰椎压缩性骨折手术患者心理健康状况及对并发症和椎体功能恢复的影响。方法:选取2018年1~12月于南阳医学高等专科学校第一附属医院骨科诊治的胸腰椎压缩性骨折患者87例,采取抑郁自评量表(SDS)和焦虑自评量表(SAS)对患者的心理健康状况进行调查,自制并发症调查问卷调查并发症发生情况,以腰椎功能量表(JOA)调查椎体功能恢复情况。结果:患者SDS评分为(56.34±11.56)分,有抑郁者24例,发生率为27.59%,SAS评分为(53.48±10.39)分,有焦虑者37例,发生率为42.53%;43例发生并发症,发生率49.43%,抑郁患者并发症发生率为66.67%,高于无抑郁患者42.86%,焦虑患者并发症发生率为62.16%,高于无焦虑患者40.00%,差异均有统计学意义(均P<0.05);抑郁患者JOA评分为(16.80±2.35)分,低于无抑郁患者(18.39±2.13)分;焦虑患者JOA评分为(16.88±2.40)分,低于无焦虑患者(18.91±2.94)分,差异均有统计学意义(均P<0.05)。结论:胸腰椎压缩性骨折手术患者心理健康相对较差,患者负性情绪会影响到患者并发症及椎体功能的恢复,建议对患者采取必要的心理干预,以改善预后。
Objective To explore the mental health status of patients undergoing thoracolumbar compression fracture surgery and its influence on complications and vertebral function recovery.Methods A total of 87 patients with thoracolumbar compression fractures treated in the department of orthopaedics of the First Affiliated Hospital of Nanyang Medical College from January to December 2018 were selected as the research objects.The mental health status of patients was investigated by Self-Rating Depression Scale(SDS)and Self-Rating Anxiety Scale(SAS),the incidence of complications was investigated by self-made complication questionnaire,and the recovery of vertebral function was investigated by Japanese Orthopaedic Association Scores(JOA).Results The SDS score of tlie patients was(56.34±11.56),and there were 24 patients with depression,with an incidence of 27.59%.The SAS score was(53.48±10.39),and there were 37 patients with anxiety,with an incidence of 42.53%.Complications occurred in 43 cases,with an incidence of 49.43%.The complication rate of depressed patients was 66.67%,which was higher than that of non-depressed patients(42.86%).The complication rate of anxiety patients was 62.16%,which was higher than that of non-anxiety patients(40.0%),and the differences were statistically significant(P<0.05).Patients with depression had a JOA score of(16.80±2.35),which was lower than(18.39±2.13)for those without depression.The JOA score of(16.88±2.40)for anxious patients was lower than(18.91±2.94)for those without anxiety.All the differences were statistically significant(P<0.05).Conclusions The psychological health of patients undergoing thoracolum-lumbar compression fracture surgery is relatively poor,and the negative emotions of patients will affect the complications and the recovery of vertebral function,so it is suggested that necessary psychological intervention should be taken to patients to improve the prognosis.
