危机管理模式在院前急救中的应用效果

The application effect of crisis management mode in pre-hospital emergency

目的观察评价分析危机管理模式(CMM)在院前急救(PHE)中的应用效果。方法选取2017年1月~2019年12月南方医科大学南海医院急诊科PHE诊治的患者1218例,所有患者随机对照试验(RCT)分为两组,对照组609例给予PHE常规处理措施;观察组609例在对照组基础上给予CMM,并比较两组PHE后的临床效果。结果观察组抢救成功率为98.85%,明显高于对照组的92.12%(χ^2=32.009,P <0.05),而危重24h死亡率为0.99%、意外事件发生率为2.96%,显著低于对照组的2.79%、7.55%(χ^2=5.362、12.929,P <0.05);观察组护理总满意度为98.52%,明显高于对照组的92.94%(χ^2=23.222,P=0.000)。结论 CMM通过在PHE工作中大大地提高患者抢救成功率,且能够显著地降低患者在24h内死亡事件及意外事件发生情况,有效提高患者或其家属的护理满意度,值得在PHE急诊科大力推广应用。

Objective To observe, evaluate and analyze the effect of crisis management model(CMM) in prehospital first aid(PHE). Methods From January 2017 to December 2019, a total of 1218 patients were diagnosed and treated in the emergency department of Nanhai Hospital of Southern Medical University. All patients were divided into two groups in a randomized controlled trial(RCT). The control group 609 cases were given to the hospital Prehospital first aid(PHE) routine treatment measures;609 cases in the observation group were given crisis management mode(CMM) on the basis of the control group, and the clinical effects after pre-hospital first aid(PHE) were compared between the two groups. Results The rescue success rate in the observation group was 98.85%, which was significantly higher than 92.12% in the control group(χ^2=32.009, P < 0.05), while the critical 24 h mortality rate was 0.99% and the accident rate 2.96% was significantly lower than that in the control group(2.79%, 7.55%)(χ^2=5.362, 12.929, P < 0.05);98.52% of total nursing satisfaction in the observation group was significantly higher than 92.94% in the control group(χ^2=23.222, P=0.000). Conclusion Crisis management mode(CMM) can effectively improve the success rate of patients in pre-hospital emergency(PHE) work, and can significantly reduce the incidence of deaths and accidents in patients within 24 hours, and effectively improve patients’ nursing satisfaction of family members is worth promoting and applying in the emergency department of PHE.
