急诊科手法复位外固定治疗无骨折踝关节脱位的效果及对疼痛程度、患者满意度的影响

The effect of manual reduction and external fixation in emergency department on the dislocation of ankle joint without fracture and the influence on the degree of pain and satisfaction

目的探讨急诊科手法复位外固定治疗踝关节脱位(无骨折)的效果及对疼痛程度、患者满意度的影响。方法回顾性分析2018年1月~2020年1月本院80例踝关节脱位(无骨折)患者的临床资料,均行急诊科手法复位外固定。分析即刻复位效果、复位后不同时段踝关节功能评分及视觉模拟评分法(VAS),统计治疗满意度。结果复位后即刻,80例患者解剖复位率为93.75%;患者复位后即刻及复位后1、4、8、12周时VAS评分均低于入院时,且呈下降趋势(P <0.05);患者复位后即刻及复位后1、4、8、12周时踝关节功能评分均高于入院时,且总评分呈增高趋势(P <0.05);患者满意度为95.00%。结论急诊科手法复位外固定治疗踝关节脱位(无骨折)可获得较好复位效果,缓解疼痛,促进关节功能恢复,提升患者满意度。

Objective To explore the effect of manual reduction and external fixation in emergency department on the dislocation of ankle joint without fracture and the influence on the degree of pain and potients satisfaction. Methods The clinical data of 80 patients with the dislocation of ankle joint without fracture in our hospital from January 2018 to January 2020 were retrospectively analyzed, and all of them were treated with manual reduction and external fixation in emergency department. The effect of immediate reduction, the ankle joint function scores and the visual analogue scale(VAS) scores at different periods after reduction were analyzed, and the degree of satisfaction with treatment was counted. Results Immediately after reduction, the anatomical reduction rate of the 80 patients was 93.75%. The VAS scores of the patients immediately after reduction and at 1, 4, 8 and 12 w after reduction were all lower than those at admission, and showed a downward trend(P < 0.05). The ankle joint function scores of the patients immediately after reduction and at 1, 4, 8 and 12 w after reduction were all higher than those at admission, and the total scores showed an upward trend(P < 0.05). The degree of satisfaction among the patients was 95.00%. Conclusion In the treatment of the dislocation of ankle joint without fracture, manual reduction and external fixation in emergency department can achieve better effect of reduction, relieve pain, promote the recovery of joint function and improve the degree of satisfaction among the patients.
