不同负压吸引人流术后即时宫内节育器放置对受术者意外妊娠及因症取出的影响

Effect of Different Negative Pressure Suction Induced Abortion Immediate Intrauterine Device Placement on Unexpected Pregnancy and Symptomatic Removal

目的探究不同负压吸引人流术后即时宫内节育器(IUD)放置对受术者意外妊娠及因症取出的影响。方法选取2018年1月至2020年1月无锡市第八人民医院行负压吸引人流术并要求术后放置节育器的80例患者作为研究对象,采用随机数字表法分为对照组[n=40,实施电动负压吸引人工流产术(EVA)后即时放置IUD]和观察组[n=40,实施手动负压吸引人工流产术(MVA)后即时放置IUD],比较两组手术相关指标(手术时间、术中出血量、术后出血时间),比较术后6个月内两组避孕效果、IUD放置情况(意外妊娠率、IUD位置异常率、IUD因症取出率)及并发症发生情况。结果观察组手术时间、术后出血时间均显著短于对照组(P<0.05),观察组术中出血量显著低于对照组(P<0.05)。术后6个月内,两组意外妊娠率差异无统计学意义(P>0.05),观察组IUD位置异常率、IUD因症取出率、并发症发生率显著低于对照组(P<0.05)。结论相较于实施EVA,MVA后即时放置IUD操作更为精简,对患者子宫损伤更小,且可有效改善IUD放置情况,有利于减少并发症发生。

Objective To explore the effect of different negative pressure suction on the unexpected pregnancy and removal of IUD.Methods A total of 80 patients with negative pressure suction induced abortion and required to place IUD after operation in Wuxi No.8 People's Hospital from January 2018 to January 2020 were selected as the research objects.They were randomLy divided into control group[n=40,placing IUD immediately after implementing electric negative pressure suction induced abortion(EVA)]and observation group[n=40,placing IUD immediately after manual negative pressure suction induced abortion(MVA)].The contraceptive effect,IUD placement(unintended pregnancy rate,IUD abnormal position rate,IUD removal rate)and complications were compared between the two groups within 6 months after operation.Results The operation time and postoperative bleeding time of the observation group were significantly shorter than those of the control group(P<0.05),and the intraoperative blood loss of the observation group was significantly lower than that of the control group(P<0.05).Within 6 months after operation,there was no significant difference in the rate of unexpected pregnancy between the two groups(P>0.05).The abnormal IUD position rate,IUD removal rate and complication rate in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Compared with the implementation of EVA,the operation of placing IUD immediately after MVA is more simple,less damage to the uterus,and can effectively improve the IUD placement,which is conducive to reduce the incidence of complications.
