某院2019年主要革兰阴性杆菌分布特点及耐药性分析

Distribution Characteristics and Drug Resistance of Main Gram Negative Bacilli in A Hospital in 2019

目的探究2019年我院分离的革兰阴性杆菌分布特点及细菌耐药情况,以期指导临床合理用药。方法收集我院2019年全年各临床科室送检的各类标本中分离到致病菌株1232株,其中对前5位革兰阴性杆菌的分离情况、科室分布及细菌耐药性进分析。结果分离出前5位的分别是大肠埃希菌468株(37.99%)、肺炎克雷伯菌304株(24.68%)、铜绿假单胞菌189株(15.34%)、鲍曼不动杆菌68株(5.52%)及流感嗜血杆菌53株(4.30%);主要分布为ICU、呼吸科、儿科、肿瘤科、消化内科、泌尿外科等科室;大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌和鲍曼不动杆菌对碳青霉烯类、阿米卡星敏感度均在85%以上,流感嗜血杆菌对氨苄西林和复方西诺明的敏感性低,其他均敏感。结论不同临床科室在根据药敏报告合理用药的同时,应有自己的经验用药特点,从而提高临床疗效。

Objective To explore the distribution characteristics and drug resistance of gram-negative bacilli isolated in our hospital in 2019,so as to guide clinical rational drug use.Methods All 1232 strains of pathogenic bacteria were isolated from various clinical departments in our hospital in 2019.The isolation,department distribution and bacterial resistance of the top five gram-negative bacilli were analyzed.Results The top five strains were escherichia coli 468(37.99%),klebsiella pneumoniae 304(24.68%),pseudomonas aeruginosa 189(15.32%),acinetobacter baumannii 68(5.52%)and haemophilus influenzae 53(4.30%);the main distribution were ICU,respiratory department,pediatrics,oncology department,digestive medicine,urology and other departments;escherichia coli,klebsiella pneumoniae the sensitivity rates of pseudomonas aeruginosa and acinetobacter baumannii to carbapenems and amikacin were more than 85%.The sensitivity rates of haemophilus influenzae to ampicillin and compound sinoxamine were low,and others were sensitive.Conclusion Different clinical departments should have their own characteristics of experience in drug use,so as to improve the clinical efficacy.
