伤科黄水外敷在急性腰扭伤中的应用

Effects of Shangke Huangshui Gauze External Application in Acute Lumbar Sprain

目的观察伤科黄水外敷治疗急性腰扭伤的临床疗效。方法急性腰扭伤患者116例随机分为对照组与外敷组,对照组给予西医常规治疗和护理,外敷组给予西医常规治疗联合伤科黄水外敷,连续治疗1周。比较两组临床疗效、治疗前后中医证候积分、腰痛功能障碍调查表(RMDQ)评分、腰椎日本骨科协会评估治疗分数(JOA)、腰椎活动度(ROM)、视觉模拟(VAS)评分、Barthel指数。结果外敷组的总有效率为98.28%,高于对照组的91.38%(P<0.05);治疗后两组的中医证候积分、RMDQ评分、VAS评分均较治疗前显著降低(P<0.05),且治疗后外敷组的中医证候积分、RMDQ评分、VAS评分均显著低于对照组(P<0.05);治疗后两组的腰椎JOA评分、ROM、Barthel指数均较治疗前显著升高(P<0.05),且治疗后外敷组的腰椎JOA评分、ROM、、Barthel指数显著高于对照组(P<0.05)。结论伤科黄水外敷治疗急性腰扭伤可以缓解疼痛,改善腰椎功能,提高临床疗效及提高生活质量。

Objective:To investigate the clinical efficacy of Shangke Huangshui Gauze external application on acute lumbar sprain.Methods:A total of 116 patients with acute lumbar sprain were randomly divided into the control group and the external application group.The control group was given conventional western medicine treatment and nursing.The external application group was given conventional western medicine combined with Shangke Huangshui Gauze external application for 1 weeks.The efficacy of the two groups,TCM syndrome scores,low back pain dysfunction questionnaire(RMDQ)score,lumbar spine Japan Orthopaedic Association Evaluation Treatment Score(JOA),lumbar spine activity(ROM),visual simulation(VAS)score,Barthel index before and after treatment were compared.Results:The total effective rate of the external application group was 98.28%,which was higher than 91.38% of the control group(P<0.05).After treatment,the TCM syndrome scores,RMDQ scores,and VAS scores of the two groups were significantly lower than those before treatment(P<0.05);the TCM syndrome score,RMDQ score,and VAS score of the external application group were significantly lower than those of the control group(P<0.05).After treatment,the lumbar JOA scores,ROM,and Barthel index of the two groups were significantly higher than those before treatment(P<0.05);the lumbar JOA score,ROM,and Barthel index of the external application group were significantly higher than those of the control group(P<0.05).Conclusion:External application of Shangke Huangshui Gauze for acute lumbar sprain can relieve pain,improve lumbar spine function,improve clinical efficacy,and improve quality of life.
