彩色多普勒超声心动图诊断肺源性心脏病的临床价值

Clinical value of color Doppler echocardiography in the diagnosis of cor pulmonale

目的研究肺源性心脏病(CPM)应用彩色多普勒超声心动图(CDE)诊断的临床价值。方法选取2018年12月~2019年12月于葫芦岛市连山区人民医院就诊治疗并行CDE检查的70例CPM患者为研究组,并选取同期行CDE检查的70例健康体检人员为对照组。比较两组的超声心动图指标测量值及肺动脉高压情况。结果研究组患者的心肌收缩期峰值纵向应变(SL)、左室质量(LVM)、左房容积(LAV)、右心室前壁厚度(RVAW)、右心室横径(RV)、右心房横径(RA)及肺动脉主干内径(MPAD)高于对照组,心肌收缩期峰值径向应变(SR)、左室射血分数(LVEF)低于对照组,差异有统计学意义(P<0.05)。研究组患者的肺动脉收缩压为(85.45±15.28)mmHg,舒张压为(42.64±9.59)mmHg,平均压为(70.84±9.44)mmHg。研究组所纳入的70例患者中做出诊断的有60例,占85.71%;未做出诊断的有10例,占14.29%。结论CDE能准确估测患者的肺动脉高压,对诊断CPM具有较高的临床价值。

Objective To study the clinical value of color Doppler echocardiography(CDE)in the diagnosis of cor pulmonale(CPM).Methods A total 70 patients with CPM who treated and accepted CDE examination in People′s Hospital of Lianshan District in Huludao City from December 2018 to December 2019 were selected as the study group,and 70 healthy physical examinees who underwent CDE during the same period were selected as the control group.The echocardiographic indexes and the pulmonary arterial hypertension of the two groups were compared.Results The myocardial systolic peak longitudinal strain(SL),left ventricular mass(LVM),left atrial volume(LAV),right ventricular anterior wall(RVAW),right ventricular(RV),right atrium(RA)and main pulmonary artery diameter(MPAD)of the study group were higher than those of the control group,the myocardial systolic peak radial strain(SR)and left ventricular ejection fraction(LVEF)were lower than those of the control group,and the differences were statistically significant(P<0.05).The systolic blood pressure,diastolic blood pressure and mean pressure of pulmonary artery in the study group were(85.45±15.28),(42.64±9.59)and(70.84±9.44)mmHg.Among the 70 cases included in the study group,60 cases were diagnosed,accounting for 85.71%and 10 cases were not diagnosed,accounting for 14.29%.Conclusion CDE can accurately estimate the pulmonary arterial hypertension in patients with CPM,and has a high clinical value in the diagnosis of CPM.
