超声联合诊断甲状腺乳头状癌颈淋巴结转移的应用分析

Application of Ultrasonography in Diagnosis of Cervical Lymph Node Metastasis of Thyroid Papillary Carcinoma

目的:探讨超声联合诊断甲状腺乳头状癌颈淋巴结转移的应用。方法:选取2018年4月-2019年12月我院收治的甲状腺乳头状癌患者74例,根据手术后的病理结果,将其分成转移组和反应组。比较两组超声结果、分析影响甲状腺乳头状癌颈淋巴结转移的多因素Lpgistic回归。结果:边界情况、时间-强度曲线下面积、血流分级、钙化情况、灌注区情况、TP、皮质回声情况、液化情况无明显差异(P>0.05),转移组RTE评分、造影分布、血流分布、淋巴结边缘形态、PI、淋巴门、L/S高于反应组(P<0.05);弹性评分、造影分布、淋巴门、血流分布、PI、淋巴结边缘形态、L/S是影响甲状腺乳头状癌颈淋巴结转移的因素(P<0.05)。结论:诊断甲状腺乳头状癌颈淋巴结转移中,超声联合超声造影及弹性成像效果比较理想,可以更好的制定相应的治疗方案,临床上应当进一步推广应用。

Objective: To explore the application of ultrasonography in diagnosing cervical lymph node metastasis of thyroid papillary carcinoma. Methods: Seventy-four patients with thyroid papillary carcinoma admitted to our hospital from April 2018 to December 2019 were selected and divided into metastatic group and reaction group according to the postoperative pathological results. The results of the two groups of ultrasound were compared and the Lpgistic regression of multiple factors affecting cervical lymph node metastasis in thyroid papillary carcinoma was analyzed. The area under the curve boundary conditions, time and strength for, grading, calcification, blood flow perfusion area, TP, cortex echo, liquefaction is no significant difference(P>0.05), group transfer RTE score, angiography, blood flow distribution, edge shape, PI, hilus lymph node, the L/S above reaction group(P<0.05);Elasticity score, radiographic distribution, lymphatic hilum, blood flow distribution, PI, marginal morphology of lymph nodes, L/S were the factors influencing cervical lymph node metastasis of papillary thyroid carcinoma(P<0.05). Conclusion: in the diagnosis of cervical lymph node metastasis of thyroid papillary carcinoma, ultrasound combined with contrast-enhanced ultrasonography and elastography have an ideal effect, and can better develop the corresponding treatment plan, which should be further popularized and applied in clinical practice.
