摘要
目的分析心脏型脂肪酸结合蛋白(H-FABP)的动态变化并评价在心肌损伤坏死模型中的早期诊断和应用价值。方法选取SD大鼠,其心电图Ⅱ导联正常,并且适应性饲养一周。将大鼠分为2组:H-FABP组20只;对照组20只。H-FABP组大鼠给予一次性多点皮下注射异丙肾上腺素10 mg/kg;对照组给予等量0.9%氯化钠,每隔24 h注射1次,连续3次,于1、3、6、8 h经左侧颈总动脉采血,并用分光光度计测定血清中H-FABP和肌钙蛋白(cTnI)的含量;末次取血完毕后,立即取出大鼠的心脏制作病理切片,在光镜下观察心肌的形态学变化。结果对照组和H-FABP组不满足球形对称(P<0.05),校正系数为0.377,校正后差异有统计学意义(P<0.05),不同时间点总体间差异具有统计学意义(P<0.05),且时间与分组存在交互效应(P<0.05)。对照组与H-FABP组在1h比较差异无统计学意义(P>0.05),而在3、6、8 h比较差异有统计学意义(P<0.05)。光镜下发现H-FABP组心肌肌纤维断裂和细胞水肿,伴有组织坏死和炎症细胞的浸润。结论 H-FABP可作为异丙肾上腺素制作的大鼠心肌损伤坏死模型中的诊断指标,在急性心肌损伤的早期具有较高的诊断能力。
Objective To analyze the value of the dynamic variety of heart-type fatty acid binding protein in rat myocardial injury model.Methods The SD rats were normal were divided into control group(n=20)and H-FABP group(n=20).The rats in H-FABP group were given a dispole mult subcutaneoun injection of isoprenaline 10 mg/kg.The control group was given the same dose of 0.9%saline and injected once every 24 hours and three times.The blood was taken from left carotid artery at 1h,3h,6h,8h for testing the change of cTnI and H-FABP,and then took the heart and kept it in formalin solution for making HE histopathology slices.Results The expressions of H-FABP in two groups did not satisfied sphericity test,and Greenhouse-Geisser was 0.377(P<0.05).After correction,the difference was statistically significant(P<0.05).There were significantly differences among different time points overall between two groups(P<0.05).There was interaction between time and group(P<0.05).Compared with control group,there was no statistically significant difference at 1h,but there were significant differences at 3h,6h and 8h.The myocardial fiber breakage,tissue patchy necrosis and myocardial cell edema was are found by light microscope,and inflammatory cell infiltration were found in H-FABP group.Conclusion H-FABP can be used as a diagnostic indicator in rats myocardial injury necrosis produced by isoproterenol.It has higher diagnostic capability in the early stage of acute myocardial injury.
