未化疗乳腺癌三维超声特征与异质性评价指标的相关性分析 被引量:2

摘要 目的探讨未化疗乳腺癌三维超声像图特征与孕激素受体(PR)、雌激素受体(ER)、增殖细胞核抗原Ki67及人类表皮生长因子受体2(Her-2)等异质性评价指标的内在关系。方法选取单侧单发的未经化疗的乳腺癌患者231例,术前均行三维自动乳腺超声检查,术后获取异质性评价指标相关的生物学标记物结果,分析肿瘤三维超声特征(肿瘤部位、肿块最大径、形态、边缘、微钙化、冠状面汇聚征及跳跃征)与异质性评价指标的相关性。结果未经化疗乳腺癌三维超声特征:形态在PR、ER高低表达组间差异有统计学意义(P<0.05);肿块最大径在ER、增殖细胞核抗原Ki67、Her-2高低表达组间差异有统计学意义(P<0.05);肿块内微钙化在Her-2高低表达组间差异有统计学意义(P<0.05);冠状面汇聚征在PR、eR、增殖细胞核抗原Ki67高低表达组间差异有统计学意义(P<0.05);而肿瘤部位、边缘(是否光整)及冠状面跳跃征在PR、ER、增殖细胞核抗原Ki67及Her-2的高低表达组间差异无统计学意义(P>0.05)。结论乳腺恶性肿瘤三维超声特征(形态不规则、肿块最大径、肿块内微钙化及冠状面汇聚征)与异质性评价指标有一定的相关性。 Objective To investigate the relationship between the imaging features of three-dimensional ultrasound and ER,PR,Her-2,Ki67 in breast cancer without chemotherapy.Methods 231 patients with unilateral single breast cancer without chemotherapy were selected.Three dimensional automatic breast ultrasound examination was performed before operation.The results of biological markers related to heterogeneity evaluation indexes were obtained after operation.The correlation between tumor three-dimensional ultrasound characteristics(tumor location,maximum diameter,shape,edge,microcalcification,coronal convergence sign and jump sign)and heterogeneity evaluation indexes was analyzed.Results The shape of the mass had correlation with the expression of ER and PR(P<0.05),while the tumor size with the ER,Ki67 and Her-2(P<0.05),the microcalcifications with the Hcr-2(P<0.05),the retraction phenomenon with the ER,PR and Ki67(P<0.05).However,no significant correlation were found between the skip sign,location,margin and the expression of ER,PR,Her-2 and Ki67(P>0.05).Conclusions The features of three-dimensional ultrasound,irregular shape,size,microcalcification and the retraction phenomenon,have a certain correlation with molecular markers.
