耻骨联合在直肠癌保肛术前MRI定位的价值

目的探讨耻骨联合在直肠癌术前行盆腔增强MRI定位后,指导临床选择保肛手术或非保肛手术的价值。方法回顾性分析2014年1月至2018年10月178例直肠癌患者的临床资料。结果在盆腔增强MRI图像上标记肿瘤下缘与耻骨联合上缘的关系,肿瘤下缘位于耻骨联合上缘以上者行保肛手术,其符合率为80%,肿瘤下缘位于耻骨联合上缘以下者行非保肛手术,其符合率为90%,总体符合率为82.67%。结论在盆腔增强MRI图像上标记肿瘤下缘与耻骨联合上缘的关系,可以指导临床选择手术方式。

Objective To explore the value of pubic symphysis in guiding the clinical choice of sphincter preserving operation or non sphincter preserving operation.Methods The preoperative and postoperative data of 178 patients with rectal cancer from January 2014 to October 2018 were analyzed retrospectively.Results The relationship between the inferior margin of the tumor and the superior margin of the pubic symphysis was marked on the enhanced MRI images of the pelvis.The patients with the inferior margin of the tumor above the superior margin of the pubic symphysis underwent sphincter preserving surgery,the coincidence rate was 80%(88/110).The patients with the inferior margin of the tumor below the superior margin of the pubic symphysis underwent non sphincter preserving surgery,the coincidence rate was 90%(36/40),the overall coincidence rate was 82.67%(124/150),P<0.05.Conclusion The relationship between the inferior margin of tumor and the superior margin of pubic symphysis can be used to guide the clinical choice of operation.
