绝经后女性抑郁障碍患者海马体积改变及影响因素研究 被引量:1

摘要 目的研究绝经后女性抑郁障碍患者海马体积变化及生殖激素、皮质醇、甲状腺功能水平与海马体积的关系。方法选取2017年7月~2018年12月在我院神经内科治疗的绝经后女性抑郁障碍患者57例为病例组,选取一般资料均匹配的健康体检者40例为对照组。采用ELISA法检测两组的生殖激素、皮质醇、甲状腺功能水平。采用1.5 T磁共振成像系统扫描两组海马后,用美国哈佛大学FreeSurfer v5.3.0软件计算海马体积,统计分析海马体积与各因素(年龄、黄体生成素、卵泡刺激素)的相关性。结果与对照组相比,病例组患者的皮质醇、催乳素、卵泡刺激素及黄体生成素水平均升高,差异有统计学意义(P<0.05);Pearson相关性分析显示,海马总体积与年龄、卵泡刺激素及黄体生成素水平呈负相关(P<0.01)。结论海马体积减小、黄体生成素和卵泡刺激素水平增加可能参与了绝经后女性抑郁障碍患者疾病的发生、发展过程。 Objective To study the changes of hippocampal volume and the relationship between reproductive hormone,cortisol,thyroid function and hippocampal volume in postmenopausal women with depression.Methods A total of 57 postmenopausal women with depressive disorders were selected as the case group from July 2017 to December 2018 in our hospital's neurology department,and 40 cases of health check-ups with generally matched informations were taken as the control group.ELISA was used to detect the levels of reproductive hormones,cortisol,and thyroid function in two groups.After the 1.5 T magnetic resonance imaging system scanned the hippocampus,Harvard FreeSurfer v5.3.0 software was used to calculate the hippocampal volume.The correlation between hippocampal volume and various factors(age,luteinizing hormone,and follicle stimulating hormone)were statistically analyzed.Results Compared with that of the control group,the cortisol,prolactin,follicle stimulating hormone,and luteinizing hormone in the case group was higher the difference was statistically significant(P<0.05).Pearson correlation analysis showed that the total hippocampal volume was negatively correlated with age,follicle-stimulating hormone and luteinizing hormone levels(P<0.01).Conclusion Decreased hippocampal volume and increased levels of luteinizing hormone and follicle stimulating hormone may be involved in the development of disease in women with postmenopausal depression.
