摘要 目的探讨内皮素-1(ET-1)、内皮微粒(EMP)和高敏心肌肌钙蛋白T(hs-cTnT)水平在缺血性心脏病中的意义。方法检测98例缺血性心脏病患者(缺血性心脏病组)、100名体检健康者(正常对照组)血清ET-1、EMP和hs-cTnT水平。按纽约心脏病学会(NYHA)心功能分级标准将98例患者分为NYHAⅠ级27例、NYHAⅡ级34例、NYHAⅢ级25例、NYHAⅣ级12例。根据Gensini评分将98例患者分为轻度狭窄32例、中度狭窄45例和重度狭窄21例。结果缺血性心脏病组血清ET-1、EMP和hs-cTnT水平明显高于正常对照组(P<0.05)。NYHAⅣ级、NYHAⅢ级患者血清ET-1、EMP和hs-cTnT水平明显高于NYHAⅠ级、NYHAⅡ级(P<0.05),且NYHAⅣ级高于NYHAⅢ级患者(P<0.05)。轻度狭窄、中度狭窄和重度狭窄患者血清ET-1、EMP和hs-cTnT依次升高,各组间差异均有统计学意义(P<0.05)。结论ET-1、EMP和hs-cTnT水平与缺血性心脏病患者心功能分级和冠状动脉狭窄程度有一定关系。 Objective To investigate the roles of endothelin-1(ET-1),endothelial microparticle(EMP)and high-sensitivity cardiac troponin T(hs-cTnT)in patients with ischemic heart disease.Methods Serum levels of ET-1,EMP and hs-cTnT were determined in 98 patients with ischemic heart disease(ischemic heart disease group)and 100 healthy subjects(healthy control group).According to the New York Heart Association(NYHA)cardiac function classification,there were 27 patients with NYHA classⅠ,34 patients with classⅡ,25 patients with classⅢand 12 patients with classⅣ.According to Gensini score,98 patients were classified into mild stenosis(32 cases),moderate stenosis(45 cases)and severe stenosis(21 cases).Results The levels of ET-1,EMP and hs-cTnT in ischemic heart disease group were significantly higher than those in healthy control group(P<0.05).The levels of ET-1,EMP and hs-cTnT in patients with NYHA classⅣandⅢwere significantly higher than those in patients with NYHA classⅠandⅡ(P<0.05).Moreover,patients with NYHA classⅣhad higher levels of ET-1,EMP and hs-cTnT than those with NYHA classⅢ(P<0.05).The levels of ET-1,EMP and hs-cTnT in patients with mild,moderate and severe stenosis increased in turn(P<0.05).Conclusion The levels of ET-1,EMP and hs-cTnT are correlated with the grade of cardiac function and the degree of coronary artery stenosis in patients with ischemic heart disease.
