摘要
目的:观察生物反馈电刺激疗法联合盆底肌训练治疗产后盆底功能障碍性疾病患者的效果。方法:按治疗方法不同将134例产后盆底功能障碍性疾病患者分为对照组(n=64)和研究组(n=70)。对照组接受盆底肌训练治疗,研究组在对照组基础上联合生物反馈电刺激治疗。比较两组治疗前后盆底肌肌力、肌纤维疲劳度、阴道最大动态压力和盆底功能改善率。结果:治疗前,两组Ⅰ、Ⅱ类肌纤维疲劳度和阴道最大动态压力比较,差异均无统计学意义(P>0.05);治疗后,两组Ⅰ、Ⅱ类肌纤维疲劳度和阴道最大动态压力均优于治疗前,且研究组Ⅰ类肌纤维疲劳度和阴道最大动态压力均优于对照组,差异有统计学意义(P<0.05);治疗后,两组Ⅰ、Ⅱ类肌纤维肌力≥Ⅲ级者占比均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05);研究组尿失禁改善率和子宫脱垂改善率均高于对照组,差异有统计学意义(P<0.05)。结论:生物反馈电刺激疗法联合盆底肌训练治疗产后盆底功能障碍性疾病患者可提高阴道最大动态压力、尿失禁改善率、子宫脱垂改善率和Ⅰ类肌纤维肌力≥Ⅲ级者占比,以及降低Ⅰ类肌纤维疲劳度的效果优于单纯盆底肌训练治疗效果。
Objective:To explore effects of biofeedback electrical stimulation therapy combined with pelvic floor muscle training on patients with postpartum pelvic floor dysfunction.Methods:According to different treatment methods,134 patients with postpartum pelvic floor dysfunction were divided into control group(n=64)and study group(n=70).The control group received pelvic floor muscle training treatment,while the study group was given biofeedback electrical stimulation on the basis of that of the control group.The pelvic floor muscle strength,the muscle fiber fatigue,the maximum dynamic vaginal pressure and the improvement rate of the pelvic floor function were compared between the two groups before and after the treatment.Results:Before the treatment,there were no statistically significant differences between the two groups in the type I and II muscle fiber fatigue degrees and the maximum vaginal dynamic pressure(P>0.05).After the treatment,the type I and II muscle fiber fatigue degrees and maximum vaginal dynamic pressures of the two groups were better than those before the treatment;the type I muscle fiber fatigue degree and the maximum vaginal dynamic pressure of the study group were better than those of the control group;and the differences were statistically significant(P<0.05).After the treatment,the proportions of the two groups of type I and II muscle fibers with muscle strength≥3 were higher than those before the treatment;that of the study group was higher than that of the control group;and the differences were statistically significant(P<0.05).The improvement rates of urinary incontinence and uterine prolapse in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:Biofeedback electrical stimulation therapy combined with pelvic floor muscle training can improve the maximum vaginal dynamic pressure,the improvement rate of urinary incontinence,the improvement rate of uterine prolapse,and the proportion of those with type I muscle fiber with muscle strength≥3 in the treatment of postpartum pelvic floor dysfunction,and can reduce the type I muscle fiber fatigue degree.Moreover,it is superior to single pelvic floor muscle training.
