电针联合生物反馈治疗初产妇经阴道分娩后压力性尿失禁的临床效果

Clinical Effect of Electroacupuncture Combined with Biofeedback in the Treatment of Stress Urinary Incontinence After Vaginal Delivery of Primipara

摘要 目的研究电针联合盆底肌电生物反馈治疗压力性尿失禁的的临床疗效。方法选取2018年1月~2020年5月就诊于云南省中医医院妇科的94例初产妇经阴道分娩后压力性尿失禁患者,随机分为对照组(n=30)、电针组(n=31)和观察组(n=33)。对照组接受盆底肌功能训练,电针组在对照组基础上加用电针治疗,观察组在电针组基础上加用盆底肌电生物反馈治疗。比较三组患者ICIQ-SF、I-QOL评分;治疗前后24 h排尿次数、24 h尿失禁次数、每次排尿量、1 h尿垫实验漏尿量;尿流动力学指标。结果治疗后三组患者ICIQ-SF、I-QOL量表评分均低于治疗前,差异有统计学意义(P<0.05),观察组及电针组ICIQ-SF、I-QOL量表评分均低于对照组,观察组ICIQ-SF量表、I-QOL量表评分低于电针组,差异有统计学意义(P<0.05);治疗后三组患者24 h排尿次数、24 h尿失禁次数、1 h尿垫实验漏尿量均低于治疗前,每次排尿量高于治疗前,差异有统计学意义(P<0.05),观察组及电针组患者24 h排尿次数、24 h尿失禁次数、1 h尿垫实验漏尿量均低于对照组,每次排尿量高于对照组,差异有统计学意义(P<0.05),观察组24 h排尿次数、24 h尿失禁次数、1 h尿垫实验漏尿量低于电针组,每次排尿量高于电针组,差异有统计学意义(P<0.05);治疗后三组患者Qmax、Pdet-Qmax、MCC均高于治疗前,PVR低于治疗前,差异有统计学意义(P<0.05),观察组及电针组患者Qmax、Pdet-Qmax、MCC均高于对照组,PVR低于对照组,差异有统计学意义(P<0.05),观察组Qmax、Pdet-Qmax、MCC均高于电针组,PVR低于电针组,差异有统计学意义(P<0.05)。结论电针联合盆底肌电生物反馈治疗能有效改善SUI患者临床症状,显著提高患者的生活质量。 Objective To study the clinical efficacy of electroacupuncture combined with pelvic floor electromyography biofeedback in the treatment of stress urinary incontinence.Methods 94 patients with stress urinary incontinence after vaginal delivery in primiparas from January 2018 to May 2020 in the Gynecology Department of Yunnan Provincial Hospital of Traditional Chinese Medicine were selected and randomly divided into control group(n=30)and electroacupuncture group(n=31)and the observation group(n=33).The control group received pelvic floor muscle function training,the electroacupuncture group was treated with electroacupuncture on the basis of the control group,and the observation group was treated with pelvic electromyography biofeedback on the basis of the electroacupuncture group.The ICIQ-SF and I-QOL scores of the three groups were compared;the number of urination at 24 h before and after treatment,the number of urinary incontinence at 24 h,the volume of urine each time,the volume of urine leakage in the 1 h urine pad experiment;urodynamic indicators.Results After treatment,the ICIQ-SF and I-QOL scale scores of the three groups of patients were lower than before treatment,the difference was statistically significant(P<0.05),The scores of ICIQ-SF and I-QOL scales of the observation group and the electroacupuncture group were lower than those of the control group.The scores of the ICIQ-SF and I-QOL scales of the observation group were lower than that of the electroacupuncture group,the difference was statistically significant(P<0.05);After treatment,the three groups of patients'24 h urination frequency,24 h urinary incontinence frequency,and 1 h urine pad test leakage volume were all lower than before treatment,and each time urination volume was higher than before treatment,the difference was statistically significant(P<0.05).The observation group and electroacupuncture group patients'24 h urination frequency,24 h urinary incontinence frequency,and 1 h urine pad test leakage volume were lower than those of the control group,and the urine volume per time was higher than that of the control group,the difference was statistically significant(P<0.05).The observation group's 24 h urination frequency,24 h urinary incontinence frequency,and 1 h urine pad test leakage volume were lower than the electroacupuncture group,and the urine output per time was higher than the electroacupuncture group,the difference was statistically significant(P<0.05);After treatment,the Qmax,Pdet-Qmax and MCC of the three groups of patients were higher than before treatment,and the PVR was lower than before treatment,the difference was statistically significant(P<0.05).The Qmax,Pdet-Qmax and MCC of the observation group and the electroacupuncture group were all higher than the control group,PVR lower than the control group,the difference was statistically significant(P<0.05),the observation group Qmax,Pdet-Qmax,MCC were higher than the electroacupuncture group,PVR was lower than the electroacupuncture group,the difference was statistically significant(P<0.05).Conclusion Electroacupuncture combined with pelvic floor myoelectric biofeedback therapy can effectively improve the clinical symptoms of SUI patients and significantly improve the quality of life of patients.
